An NHS Health Board in Haverfordwest is seeking a Registered Nurse to join the Discharge Specialist Team. You will facilitate timely hospital discharges, assess and coordinate care for patients with complex needs. This role requires excellent communication and assessment skills, with the ability to work independently in a fast-paced environment. Proficiency in Welsh is desirable. Salary ranges from £39,263 to £47,280 annually, pro rata.
The Urgent and Intermediate Care team have an opening for a dynamic and experienced individual to join our Discharge Specialist Team. The team support in both community and secondary care settings to facilitate timely hospital discharges and enhance the quality and safety of the service given to individuals at home with an aim to optimise recovery and resilience.
The Discharge Specialist Team is an adaptable team with a variety of functions to support and advocate the patient in the interface between services and organisations, identifying the appropriate pathway for the patient, ensuring 'what matters to them' is at the forefront of decisions in the process.
The service aims to deliver support for discharge to recover and assess at home working in conjunction with our community multidisciplinary team and in-reaching into our hospitals.
We seek a Registered Nurse with a proven interest in the discharge process. The post holder will be based mainly in Withybush General Hospital, South Pembrokeshire Hospital and Tenby Cottage Hospital and will be responsible for assessing and coordinating the discharge of patients identified as having complex needs.
This post will require you to have a high degree of autonomy and manage your own day-to-day activity, possess excellent communication and assessment skills and ideally have experience and good knowledge of working with adult social care and community services within primary or secondary care.
The post holder will work in a fast paced, evolving environment and is expected to undertake the holistic assessments of patients using a person centred approach and co-ordinate the discharge process across both health and social care to facilitate the timely discharge of patients from the hospitals.
