What you will do:

• Conduct comprehensive assessments of adults and carers within diverse settings, collaborating with them to create plans tailored to their specific needs.
• Complete capacity assessments and Best Interest Meetings/decisions as required under the Mental Capacity Act 2005.
• Assist adults who may lack capacity, especially regarding consent to their accommodation and care needs, supporting applications to the Court of Protection for deprivation of liberty authorisation.
• Manage a caseload, delivering social work services to people with care and support needs, including those at risk of harm and neglect.
• Identify and respond appropriately to individuals who may appear to be at risk of significant harm, ensuring their safety while respecting their right to self‑determination when they have capacity.
• Collaborate within a multi‑disciplinary team to carry out assessments and planning.
• Liaise with other social service professionals, health professionals, and partner agencies.
• Undertake necessary administration, including keeping up‑to‑date records and managing your workload effectively.

Requirements:

• Registration with Social Work England.
• Degree or equivalent qualification in Social Work.
• Demonstrable UK qualified experience in social work related to adult care and discharge processes.
• Experience with capacity assessments, safeguarding, multi‑disciplinary teams, and record management.
