Job Summary

The Advanced Clinical Practitioner (ACP) will play a key role in helping to deliver our high-quality Total Triage model. As an autonomous practitioner working within a supported environment, the ACP will provide timely clinical assessment, decision‑making, and management for patients presenting through digital, telephone, and face‑to‑face access routes. The postholder will use advanced clinical skills to triage incoming clinical requests, ensure patients are directed to the most appropriate clinician or service, and provide same‑day assessment and directing to treatment where indicated.

Main duties of the job
  • Provide advanced clinical assessment and management of patients via digital, telephone, and face‑to‑face consultations.
  • Triage all incoming clinical requests, ensuring patients are directed to the most appropriate clinician, service, or self‑care option.
  • Deliver same‑day urgent care, including diagnosis, treatment, prescribing, and safety‑netting within scope of practice.
Consultations following triage
  • Conduct timely clinical assessments of patients who have been triaged and allocated for ACP review, using telephone, video, or face‑to‑face appointments as appropriate.
  • Clarify and explore presenting complaints, ensuring the initial triage information is validated and any red flags or risk factors are identified.
  • Perform advanced clinical examinations and diagnostic reasoning to establish working diagnoses and determine safe, effective management plans.
  • Provide treatment, prescribing, and interventions within scope of practice for both acute and long‑term conditions.
  • Offer safety‑netting advice, escalation plans, and follow‑up arrangements, tailored to individual patient needs.

Parkside Surgery is a well‑established, busy GP practice serving the Burnley community. It sits at the heart of local primary care provision under the Prestige Medical Group, providing comprehensive general medical services to over sixteen thousand patients across the area.

Our commitment to comprehensive care from acute presentations to chronic disease management, minor procedures, and preventative health offers the right ACP the opportunity to work at the top of their scope, enjoy variety in their caseload, and contribute meaningfully to service delivery and patient outcomes.

Job responsibilities
  • Practice in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions at this level of practice.
  • Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information.
  • Act on professional judgement about when to seek help, demonstrating critical reflection on own practice, self‑awareness, emotional intelligence, and openness to change.
  • Work in partnership with individuals, families and carers, using a range of assessment methods as appropriate.
  • Demonstrate effective communication skills, supporting people in making decisions, planning care or seeking to make a positive change.
  • Use expertise and decision‑making skills to inform clinical reasoning.
  • Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care.
  • Exercise professional judgement to manage risk.
  • Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice.
  • Implement and evaluate individual specialised treatment plans for chronic disease patients.
  • Identify, manage and support patients at risk of developing long‑term conditions, preventing adverse effects to the patients health.
  • Be a professional role model for the organisation.
  • Actively engage in peer review to inform own and others practice, formulating and implementing strategies to act on learning and make improvement.
  • Actively seek feedback and involvement from individuals, families, carers, communities and colleagues in the co‑production of service improvements.
  • Critically assess and address own learning needs, negotiating a personal development plan that reflects the breadth of ongoing professional development across the four pillars of advanced clinical practice (clinical practice, leadership and management, education and research).
  • Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
  • Maintain accurate clinical records in conjunction with extant legislation and to ensure SNOMED CT codes are used effectively.
  • Critically appraise and synthesise the outcome of relevant research, evaluation and audit, using the results to underpin own practice and to inform that of others.
  • Develop and implement robust governance systems and contribute to practice targets (QOF, etc.), complying with local and regional guidance.
  • Chaperone patients where necessary.
  • Support the team in dealing with clinical emergencies.
  • Support the team with all safeguarding matters in accordance with local and national policies.
  • Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately.
  • Deliver opportunistic health promotion where appropriate.
  • Conduct comprehensive patient assessments, including medical history, physical examinations, and diagnostic tests.
  • Develop and implement individualized treatment plans in collaboration with the healthcare team.
  • Monitor patient progress and make necessary adjustments to treatment plans.
  • Educate patients and their families on disease management, prevention strategies, and healthy lifestyle choices.
  • Collaborate with other healthcare professionals to ensure coordinated and efficient care delivery.
  • Maintain accurate and up‑to‑date medical records.
Qualifications
  • Advanced Nurse Practitioner or Advanced Clinical Practitioner.
  • Masters – desirable.
  • Non‑Medical Prescriber.
  • Experienced in clinical triage.
  • Clinically able to manage wide range of chronic and acute presentations in adults and children.
Essential requirements
  • Minimum 30 hours.
  • Must work Fridays.
  • Will need to work until 6.30 pm 2 days.
