Job description

Would you like to work in a forward-thinking team who provides excellent specialist care to haemoglobinopathy patients?



Across the Thames Valley region there are over 700 patients (children and adults) with a haemoglobinopathy disorder. These include sickle cell anaemia, thalassaemia and other rare inherited anaemias.



The care of this group of patients and families is determined by national Haemoglobinopathy care standards (adult andpaediatricSCD and thalassaemiaguidelines). Our specialist network is peer reviewed to the Quality Review Service standards.



Recognising the essential need for pharmaceutical input in this patient group, we have newly created this pharmacist post, which will be embedded within the adult and paediatric haemoglobinopathy team. This is a fantastic opportunity for a dynamic, motivated pharmacist to develop the skills to be able to work flexibly with both adult and paediatric services, on a local and regional level. Working closely with the friendly and welcoming haemoglobinopathy team, this is an opportunity to make a genuine difference to patient care.



Flexible working applications will be considered. We will also consider job sharing with a complementary post to make up a full-time post.



Specific experience within this specialty is not essential as we will ensure you have the orientation, support, education and training required to develop within this role.

Key roles include:
• Delivery of high-quality pharmaceutical care to adult and paediatric patients with major haemoglobin disorders
• Contribution to local, national and international specialist interest groups and conferences/meetings, such as the Haemoglobinopathy Pharmacist Engagement Meeting.
• Collaboration with the haemoglobinopathies network to produce shared care agreements, guidelines, referral pathways and horizon scanning for new medicines.
• Participation in multidisciplinary ward rounds and team meetings
• Pharmacist-led patient education clinics
• Pharmacist non-medical prescribing in multidisciplinary clinics and day treatment unit

The post is hosted within the friendly adult cancer & haematology pharmacy team, which includes Consultant Pharmacists, Highly Advanced and Advanced Pharmacists, as well as the first Advanced Cancer Pharmacy Technicians nationally. You would have strong networks of peer support and mentorship. Our team have specialist haematology and oncology pharmacist non-medical prescribers who are embedded within the MDT to review and prescribe important treatments for our patients.

Clinical and Professional Practice

1. To provide a clinical pharmacy service according to Trust agreed standards including:

· Individual prescription review to optimise therapy

· Confirmation of the patient’s medication history

· Advice on dosage, side-effects, drug interactions, cautions and monitoring required

· Advise on administration of medicines

· Appropriate and clear endorsing of prescriptions to ensure safe practice

· Monitor the effect and appropriateness of medication

· Education of patients on their medication, such as conducting a patient counselling clinic to improve patient education adherence and medicines optimisation, or delivery of teaching sessions as part of a patient outreach event

· Involvement in discharge planning and provision of discharge medication for inpatients where appropriate



2. To ensure medicines are used appropriately, safely and cost-effectively in accordance with Trust policy, standard operating procedures and medicines legislation.



3. To monitor medicines use within the clinical area. This includes recording of significant clinical interventions and risk management including:

· Participation in investigating clinical incidents

· Recording significant clinical incidents/near misses

· Ensuring compliance with medicines legislation and local policies



4. To use available information to influence prescribers and ensure the most cost-effective choice of therapy.



5. To participate in ward rounds, clinical meetings or out-patient clinics as appropriate to provide pharmaceutical advice to prescribers and other health care professionals.



6. To liaise with external agencies to ensure safe, timely and appropriate supply to patients e.g. Baxter, Homecare companies etc, where appropriate.



7. To participate in clinical audits that can be used to improve the quality of patient care or develop the service.



8. To develop and facilitate implementation of treatment guidelines and patient information leaflets for use of medicines within the clinical area.



9. To tutor rotational clinical pharmacists undertaking the Diploma in Clinical Pharmacy, following formal accreditation as a tutor by Cardiff University, or other university and to tutor other pharmacists and pre-registration students in the clinical area.



10. To act as a role model and mentor for clinical pharmacy.



11. To undergo accreditation to write discharge prescriptions for patients, where appropriate.



12. To provide teaching for the pharmacy and directorate teams as required.



13. To act as knowledge base/resource for the specialist area and to keep up to date with current trends and research in the specialist area. To regularly review local prescribing practices ensuring that care is based on current evidence and research.



14. With the rest of the members of the clinical pharmacy team to support the strategic developments of the service e.g. electronic prescribing, improved discharge processes, accredited technician checking etc.



15. Actively contribute to the planning and efficient co-ordination of clinical pharmacy initiatives including discharge planning, prescribing protocols, pharmacists on consultant ward rounds etc.



16. To contribute to and implement the clinical governance plans of the clinical area and the pharmacy clinical governance plans.



17. To be involved in thedevelopment, maintenance, and review of haemoglobinopathy protocols. This includes prescribing protocols and drug administration guidelines.



18. To be involved in the preparation of new drug submissions for the Trust Medicines Management and Therapeutics Committee with Consultant colleagues. To provide resources to ensure safe implementation of new technology into clinical practice.



19. To be involved in the implementation of relevant local, network and national standards including service guidelines and technology appraisals produced by the National Institute for Health and Care Excellence.



20. To attend and contribute to Haemoglobinopathy and Haematology Pharmacy Team Meetings.



21. To ensure confidentiality is maintained at all times.



22. To be professionally accountable for actions and advice.



23. To use experience and clinical judgment to assist in problem-solving and troubleshooting.



24. To give professional support to the site operational managers.



25. To support the dispensaries by participating in the screening and checking prescriptions and dispensed items at a mutually convenient time as part of a rota (current commitment is 1-2 hours per working week and one whole weekend day every six weeks).



26. To participate in weekend and bank holiday working according to rota.



27. Any other reasonable duties as requested by the Chief Pharmacist.



28. To participate in any future 7 day working patterns should the pharmacy department change its current working practices.





Leadership, Management and Collaboration

1. To contribute to local, national and international specialist interest groups and conferences/meetings, such as the Haemoglobinopathy Pharmacist Engagement Meeting.



2. To act as a role model for other pharmacists and healthcare professionals, demonstrating skills and expertise in medication reviews and solving complex medication-related problems.



3. To collaborate with the haemoglobinopathies network to produce shared care agreements, guidelines, referral pathways and horizon scanning for new medicines.



Education, Training & Development

1. To become a non-medical prescriber as appropriate.



2. To work as a non-medical prescriber in the haemoglobinopathy service.



3. To act as a tutor/mentor for pharmacists and other healthcare professionals undertaking undergraduate and postgraduate clinical training.
