A regional healthcare provider in Havant seeks a Clinical Pharmacist to deliver high-quality pharmacy services to mental health units and community teams. The role offers flexibility without weekend work and the opportunity for split-site working across Havant and Southampton. Key responsibilities include providing clinical advice, enhancing patient safety, and supporting multidisciplinary teams. We are seeking a Clinical Pharmacist to join our well‑established, friendly team who provide clinical support to mental health units, community hospitals and community teams across Hampshire.
There is no weekend or Bank Holiday working required, and the post is flexible with respect of how hours might be worked. There is the option, also, of split site working between bases in Havant and Southampton.
The successful applicant will provide regular clinical pharmacist support at Elmleigh Mental Health Unit and at Hollybank AMH rehabilitation unit, in our Secure, Acute and Crisis division, whilst also working closely with the Portsmouth and Southeast Division pharmacy team to provide clinical cover to Older Persons Mental Health and community rehabilitation wards.
As part of the role there is the opportunity to gain experience in other specialities such as Learning Disabilities, CAMHS, adult, LD and CAMHS forensic services and End of Life.
The successful candidate will deliver an efficient, high quality, patient‑focussed, professional and well‑coordinated clinical pharmacy service and will help to identify gaps and problem areas.
The work will include provision of prescribing advice to medical teams and medicines management to the multidisciplinary teams and patients at ward/clinic level.

Your work will focus on enhancing patient safety in medicines practice including medicines reconciliation, providing clinical professional advice to prescribers as part of a multi‑disciplinary team, discharge planning, clinical audit, and teaching.
