A leading humanitarian organization in the UK is seeking a Senior Practitioner to coordinate referrals and mentor staff in delivering high-quality care. The role involves managing risk assessments, supporting individuals transitioning from hospital to home, and ensuring service continuity. Ideal candidates will have a SVQ Level 3 in Health and Social Care and experience in person-centred planning. This position offers flexible working hours, generous annual leave, and opportunities for professional development.
Location: Dundee, with travel around the community
Salary: £23,590 to £25,155 per annum pro-rata
Hours: 35 per week
Contract: Permanent
Additional Requirement: Applicants must have a full UK licence (2 years) and access to a vehicle.
Additional Requirement: Successful candidates will be required to register with the Scottish Social Security Council (SSSC) within 3 months of starting.
