Alongside our inpatient work, the Care of the Elderly department operates an Acute Frailty service based in the hospital's Accident and Emergency department. A dedicated multi-disciplinary team of Consultant Geriatrician, Specialist Frailty Practitioner Nurse, Frailty Fellow Doctor, Physiotherapist, Occupational Therapist, Therapy Assistant and Pharmacist, all work together to assess and manage older, frail patients admitted under medicine. Daily morning ward rounds by the Consultant Geriatrician are complimented by the multi-disciplinary teams review of additional patients throughout the day. The service maintains close links with community teams, the COTE virtual ward and the COTE back of house wards to optimise patient experience and reduce length of stay.
We also run a COTE Virtual Ward. Here we provide Consultant Geriatrician led care to patients in their own homes with acute medical needs. Patients can be referred to the virtual ward from the community and secondary care. The Virtual ward team includes Specialist Frailty Nurses, an Occupational Therapist and Physiotherapist, all of whom can visit patients in their own homes. The Consultant Geriatrician leads the care of the patients through two daily MDT meetings, reviews of potential admissions to the ward in the community and in hospital and home visits to assess and manage patients in the ward.
