A prominent healthcare institution in South West London is seeking an experienced Critical Care Outreach Practitioner. The successful candidate will lead a dynamic team in delivering high standards of care to acutely unwell patients. Responsibilities include responding to emergencies, teaching clinical skills, and auditing service delivery. Ideal candidates will hold a BSc in Nursing and relevant critical care qualifications, along with leadership experience. The position offers a permanent contract with competitive salary and benefits, working 37.5 hours per week.

We are looking for an enthusiastic and highly motivated nurse with critical care/acute clinical and leadership experience to join our Critical Care Outreach team (CCOT) as we expand our service to provide 24/7 cross‑site cover. The successful applicant will join a supportive and dynamic team, leading on delivering high standards of care to acutely unwell patients across our site. CCOT teams are a nurse‑led service supported by the critical care consultant team. You will have the opportunity to work both as a team and independently, identifying patients at risk of clinical deterioration or that are deteriorating and having their needs prioritised with appropriate assessment, intervention and ongoing management. Teaching and delivering education to ward teams is an essential part of the role together with leading on deteriorating patient quality improvement work across site.
