Supported Assistant Practitioner/Assistant Practitioner | 9343 | Permanent Contract | 37 hours per week | £28,598 to £30,024 per annum | (Grade F/G) | Norfolk & Norwich Hospital, Norwich

An exciting opportunity has arisen to join our Hospital Discharge Team within the Home First Hub, supporting individuals following an inpatient stay. The primary base for this role is the Norfolk and Norwich University Hospital, with responsibility for facilitating discharges across the county for Central Norfolk residents.

Our team is experienced with working with complex cases, have a sound knowledge of the Care Act (2014), Mental Capacity Act (2005) and Safeguarding, as well as excellent time management and IT skills. Any new staff are supported to develop a thorough working knowledge of Norfolk County Council processes and procedures. Covering South Norfolk, our team of Social Workers, Assistant Practitioners and Occupational Therapists use the Care Act (2014) to assess and provide support to adults living in the community. We are investing in people who can take us forward on our journey to embed our vision.

As an Assistant Practitioner within this team, you will undertake assessments, service commissioning and reassessment work. These will be carried out via video/telephone and/or face to face. You will have excellent communication, record‑keeping and keyboard skills, along with experience of working with vulnerable people. In addition, you should have a commitment to excellent customer service and be familiar with the Departments' eligibility criteria. In return, we can offer you the opportunity to work in an exciting and challenging area of Social Care, where you will be supported to develop into the role with regular supervision, training and peer support.

Our team operates a 7‑day service, so occasional weekend working is required (approximately once per month). The role is based within the hospitals discharge services, where you will carry out face‑to‑face assessments in a ward environment.
