Job description

Due to changes in the ARRS Scheme to introduce General Practitioners, Stockport East and South PCN are looking to expand our team by seeking to employ a highlymotivated, newly qualified General Practitioner to work alongside our practicesand PCN staff.

This is an exciting opportunity to join our multi-disciplinary PCN team consisting of 8 GP Practices, Clinical Pharmacists, Paramedics, Social Prescribers, Physiotherapists, Care Coordinators, Health Coach and Pharmacist Technicians.

SESSIONS: We will consider both full-time of up to 8 sessions per week, or part-time posts of at least 4 sessions a week.

PLACE OF WORK:Stockport South and East PCN.

EMPLOYER: Practice within the PCN.

CONTRACT: 6 month fixed term contract and subject to the funding related to the ARRS role.

Main duties of the job

To perform as an autonomous general practitioner workingindependently and with other health care professionals to assess, diagnose andtreat the conditions of patients attending within a primary care setting, bothin practice and on home visits.

Provide expert professional advice to patients, carers andcolleagues and ensure the maintenance of clinical excellence. To beprofessionally accountable and responsible for all elements of clinicalpractice in accordance with the GMC code of conduct ensuring the safety ofpatients and quality of their care is primary.

The post holder will need to have full access to a car toenable full geographical working across Stockport East and South PCN and workflexibly to meet the needs of the PCN.

Stockport East and South PCN is a dynamic, forward thinking PCN who are passionateabout developing and delivering excellent quality local services to meet theneeds of our patients. We are a high achieving PCN performing well across theboard with consistently positive friends and family feedback, leading in covidand flu vaccinations and managing prescribing targets well when compared withStockport as a locality and Greater Manchester as an ICB.

We are 8 GP Practices in Stockport East and South PCN. Wehave a weighted list size of approximately 60,000 patients between the 8 GPpractices. Our practices look after patients from Hazel Grove to High Lane,Marple, Romiley and Woodley.

Job responsibilities

  • In accordance with the practice rota, as agreed,the post-holder will make him/her-self available to undertake a variety ofduties including surgery consultations, telephone consultations and queries,visiting patients at home, checking and signing/authorising repeatprescriptions and dealing with queries, paperwork and correspondence in atimely fashion.
  • Making professional, autonomous decisions inrelation to presenting problems, whether self-referred or referred from otherhealth care workers within the organisation
  • Assessing the health care needs of patients withundifferentiated and undiagnosed problems
  • Screening patients for disease risk factors andearly signs of illness
  • In consultation with patients and in line withcurrent practice disease management protocols, developing care plans for health
  • Providing counselling and health education
  • Referring to other care providers as appropriate
  • Recording clear and contemporaneous consultationnotes to agreed standards
  • Compiling and issuing computer-generated acuteand repeat prescriptions

  • Regular communication withthe health professional and managerial and administrative staff of the Practiceto ensure the smooth running of the practice.
  • Liaison with the members ofthe wide multi-disciplinary team, including ICB, hospitals, community servicesand other agencies (such as local authority, social services and voluntaryagencies) in order to assure appropriate care is provided for the Practicepopulation.
  • Communicate effectively withpatients and carers, recognising their needs for alternative methods ofcommunication.
  • Demonstrate sensitivecommunication styles to ensure patients are fully informed and consent totreatment.
  • To communicate difficultoften unpleasant or sensitive messages to patients and families.
  • Participate in Practice / PCN Meetings and others asappropriate.
Teamwork:

Responsibility for Information Resources:

  • Review and process data usingaccurate SNOMED coding in order to ensure easy and accurate informationretrieval for monitoring and auditing purposes.
  • Understand and follow therequirements of confidentiality, including the Data Protection and the Freedomof Information Act and to refer on any queries as appropriate.
  • Follow Practice policy regarding the use of email andcomputer tasks as the main non-verbal internal method of communication and useof the intranet and the internet as the main source of internal and externalinformation.

Other responsibilities within the Organisation:

  • Contribute to the developmentand achievement of the PCN strategy and goals.
  • Participate in and promotethe practice online services to patients (eg online medical viewer), along withinvolvement of practice online consultation system.
