A charitable hospice organization in Southend-on-Sea is seeking a full-time Care Co-ordinator to join their Fair Havens Community Team. The role requires strong data collection and communication skills, as well as the ability to navigate challenging situations in support of patients with palliative care needs. Applicants must be available for a 7-day service, including weekends and holidays. Join us in making every day count for those in need.
We have an opportunity for a full-time Care Co-ordinator to join our Fair Havens Community Team, located at our Fair Havens Hospice, adjacent to beautiful Priory Park in Southend on Sea.
As this role supports a 7-day service, there is a requirement that candidates be available to work on a rostered basis, across the 7 days, including weekends and bank holidays.

You will work as part of the wider team of coordinators within our Fair Havens Community Team, to ensure that our processes and HRAS referrals are streamlined with robust internal pathways, as well as undertaking administrative duties as required.

Our specialist Care Teams can support them and their family living throughout illness, death and bereavement in the comfort of their own home and through our hospice services, Fair Havens and Little Havens.
