Job Summary :

The Registered / Licensed Practical Nurse is responsible for providing and managing client and family-centered, evidence-based, efficient nursing care to individuals and families in their homes and community. This includes ensuring care within the standards and scope set by VON and the provincial regulatory body, and includes safe teaching, assignment, and delegation of tasks to other workers.

Our visiting nurses deliver client care in the client's home, including physical assessments, symptom management, palliative care, rehabilitation, wound care, education, and counseling.

Note : On-call shifts are required. Applications from current bargaining unit members will be considered first.

Key Responsibilities :

• Provide nursing care to clients with less complex needs following an established care plan; consult with RNs for more complex cases.
• Manage and evaluate care via established plans and assess client responses.
• Perform comprehensive assessments of clients and their environment.
• Gather information from clients, families, and healthcare team members.
• Document all relevant information accurately per VON standards.
• Describe and document assessment findings and reactions to care.
• Identify problems and focus care accordingly.
• Contribute to care planning in collaboration with the healthcare team.
• Implement strategies from care plans in consultation with clients and caregivers.
• Document and rationalize any modifications to the care plan.
• Incorporate current research into practice.
• Provide rationales for referrals, visit adjustments, and discharges.
• Utilize resources effectively and assign care appropriately.
• Refer clients to other providers as needed.
• Promote client independence through teaching and support.
• Share knowledge to empower clients and families.
• Use adult learning principles and established materials for health education.
• Engage families in health decisions.
• Participate in policy development and standards through professional involvement.
• Practice within quality, risk management, and safety frameworks.
• Plan, evaluate, and adjust care based on client feedback to ensure quality service.
• Collaborate with management and colleagues, maintaining competence through ongoing education.

External and Internal Relationships :

• Work with multidisciplinary teams and mentor other care providers.
• Engage with clients, families, and caregivers, providing education and support.
• Participate in community and professional organizations to promote best practices.

Education, Designations and Experience :

• Must hold a license as a Registered or Licensed Practical Nurse in the province.
• Additional certifications may be required for specialized practice areas.
• Clinical practice experience is preferred; community health experience is an asset.

• Personal vehicle, valid driver’s license, and proof of insurance.
• Criminal Records Check and possibly Vulnerable Sector Search / Child Abuse Registry Check.
• Flexibility to work various hours.
• Ability to wear PPE.

Physical demands include walking, sitting, standing, climbing stairs, and manual tasks requiring fine motor skills, in a dynamic environment.
