Job Summary

The Registered Nurse provides client and family‑centered, evidence‑based nursing care to individuals and families in homes and the community. Care is delivered within the framework of quality & risk management and the standards set by VON, provincial regulatory bodies, and Community Health Nurses of Canada. The RN safely teaches, delegates, and assigns tasks to workers of all categories.

Key Responsibilities
  • Manages and coordinates care; evaluates health outcomes; educates, counsels and advocates for individuals to meet health goals.
  • Provides nursing care for stable to highly complex clients with acute, rapidly changing needs.
  • Offers cost‑effective, organized, outcome‑directed care and delegates tasks as needed.
  • Applies VON and provincial standards in all practice.
  • Conducts comprehensive assessments of clients, families and caregivers, including their environment.
  • Develops client‑based care plans, incorporating discharge planning.
  • Implements nursing interventions per the care plan.
  • Performs technical skills safely, in line with VON policies and nursing principles.
  • Documents nursing processes clearly, concisely, accurately and appropriately per regulatory and VON standards.
  • Documents responses to the care plan and revises the plan based on realistic outcomes.
  • Promotes client, family, or caregiver involvement in health decisions and service planning.
  • Supports independence through health promotion and teaching.
  • Deals with clients and families to ensure effective delivery of care while respecting choices.
  • Integrates current research findings into clinical practice.
  • Communicates client status regularly with the health care team.
  • Provides referrals as indicated and documents rationale for changes in visits or discharge.
  • Utilizes tools and resources, managing them effectively.
  • Collaborates across a multidisciplinary team, serving as a clinical resource and mentor.
  • Encourages client‑directed care, self‑care and independence.
  • Applies adult learning principles and teaching materials to health education.

  • Engages in self‑reflection and continuing education to maintain competence.

Education, Designations and Experience
  • Must be licensed as a Registered Nurse in the province of practice.
  • Canadian Nurses Association certification in Community Health Nursing is an asset.
  • Additional degrees or certificates may be required for specialized practice.
  • Clinical practice experience is an asset.
  • Experience in community health is an asset.

Working Conditions and Physical Demands

This role requires a detail‑oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling and other movements emphasizing good body mechanics. Individuals must walk, sit, stand and climb stairs throughout the day, with some tasks requiring fine hand movements.
