Baycrest Hospital seeks a Registered Practical Nurse for its Transitional Care Unit to provide professional nursing care. This role focuses on patient preparation for discharge, ensuring wellness and functionality. Applicants should hold registration from the College of Nurses of Ontario and possess excellent communication skills.

REGISTERED PRACTICAL NURSE (RPN)
Transitional Care Unit (TCU) (3W)
Shift Type: Day, Evenings, Alt Weekends (subject to change)
Hours of Work: 7.5 hrs /shift
The Transitional Care unit (TCU) provides care for patients who are awaiting discharge to an alternative level of care setting and prepare them for discharge.The philosophy of the unit is to maintain health, wellness and function while awaiting placement in an appropriate client centered environment.
