Working as part of the interdisciplinary team as the RPN, you will work to your full scope of practice in collaboration with Nurses across the organization and with the Advance Practice Lead. You will assist in the coordination and delivery of holistic health care to residents within a supportive housing model, addressing physical, mental, emotional, social, and spiritual components of health. Modeling person-centered and evidence-based care principles, you will work within an interdisciplinary team to serve individuals experiencing long-term housing instability or hospitalizations. 40 hours/week. The program operates 7 days a week, including public holidays. Flexibility for evenings and weekends is required, along with participation in an on-call rotation involving 24/7 response with a work cell phone for one week at a time.
