Working as part of the interdisciplinary team as the RPN, you will work to your full scope of practice in collaboration with Nurses across the organization and with the Advance Practice Lead. You will assist in the coordination and delivery of holistic health care to residents within a supportive housing model, addressing physical, mental, emotional, social, and spiritual components of health. Modeling person-centered and evidence-based care principles, you will work within an interdisciplinary team to serve individuals experiencing long-term housing instability or hospitalizations. 40 hours/week. The program operates 7 days a week, including public holidays. Flexibility for evenings and weekends is required, along with participation in an on-call rotation involving 24/7 response with a work cell phone for one week at a time.

• Participate and contribute to the interdisciplinary team, maintaining positive communication and updating on community opportunities and best practices
• Provide health education, oversee medication support, and assist in staff, student, and volunteer orientation
• Support residents' health and housing stability through relationship building, assessments, care planning, and crisis intervention
• Lead and coordinate care plans, liaise with external providers, and support community nursing supports
• Ensure safety, maintain health records, and support residents' engagement in various activities

• Show initiative, lead internal teams, and foster external partnerships
• Promote a positive workplace culture and support ongoing learning

• Maintain a safe environment, adhere to policies, respond to emergencies, and participate in training

• Completion of an approved RPN program and current registration with the College of Nurses of Ontario
• Experience in community nursing, mental health, and addiction services preferred
• Strong communication, crisis intervention, and organizational skills
• Valid G-class driver’s license and own vehicle required
