Whether you work in our office, in the community, or a health care facility – you will play a lead role in providing connected, accessible, patient-centered care – and be supported by our collaborative team that includes over 9,000 regulated health care and other professionals.

As a valued team member, your mission will be to help our patients be healthier at home, while you benefit from our supports for professional growth, personal wellness, and work-life balance.

What will you do?

  • Respond to referrals from Care Coordinators to properly administer nutritional and dietetic services to patients.
  • Assess patients to identify their nutritional needs and determine patient-centered treatment goals.

  • Oversee patient progress and adjust treatment programs when needed, to ensure nutrition goals are addressed.
  • Educate and counsel patients, their families, caregivers, and/or SDMs, regarding nutritional conditions/diseases, patient issues/concerns, and dietary strategies to improve function, compensate for difficulties, and promote nutritional health.
  • Manage service deliveries in conjunction with the Care Coordinator, their families, caregivers, and/or SDMs using all available forms of communication.
  • Link patients with other in-house services or outside organizations.
  • Balance patient needs and choices with available resources, ensuring patients’ values and preferences are respected.

Accurately document and chart patient’s assessment results, goals, treatment plans, and interventions.

  • Good understanding of the roles of other health care professionals and complementary therapies.
  • Knowledge of funding agencies available to assist patients in accessing needed support services and other community resources, treatment centers, and organizations.
  • Practical knowledge and understanding of pertinent legislation.
  • Comprehensive knowledge of the standards of practice and professional guidelines set forth by CDO.
  • Ability to prioritize professional duties and manage caseload in a time efficient manner.
