Physiotherapist

We are looking for an experienced Physiotherapist to join the Worcester City neighbourhood team physiotherapists for 6 months fixed term to cover maternity leave. This is an ideal opportunity for physiotherapists interested in developing their career and skills in the community setting. We have 22.5 hours available for band 5 however we are advertising separately for a band 6 as we are open to receive applications for both. The banding will be determined by the successful applicant.

The community physiotherapy teamwork Monday to Friday between the hours of 08.00 17.00 however there is scope for flexibility.

We are a busy team with a friendly and supportive culture. The successful candidate will work autonomously however there is always a member of the team willing to assist as needed, with advice over the phone or through a joint visit. You will be supported during the induction phase to ensure you feel confident and have a clear understanding of how the team and wider MDT work to facilitate rehabilitation in the community setting.

This is a fixed term post to provide cover for absence for specific purpose that we anticipate will be required for 6 months. We reserve the right to end this contract sooner with appropriate notice.

Main duties of the job

Delivering a high standard of physiotherapy assessment and treatment to patients in their own home.

Provide advice on the rehabilitation management of patients to families, carers and other members of the MDT.

Be responsible for managing your caseload, ensuring all patients have appropriate patient centred goals set, rehabilitation plans in place and follow up reviews diarised.

Be responsible for the triage of new referrals on your given day as part of the team rota.

Assess priority patients as part of the urgent community response initiative and admission avoidance, as well as supporting hospital discharges including trauma and orthopaedic brace management and planning domiciliary interventions to facilitate independent living.

Treat a wide variety of presentations including falls, orthopaedics, neurological conditions, musculoskeletal and respiratory patients.

Maintain accurate electronic patient records in line with documentation guidelines.

Delegate and supervise appropriate tasks and treatment plans to the physiotherapy assistants.

Take part in the supervision of physiotherapy students as well as supporting the physiotherapy assistants.

Ordering of patient equipment and ensuring the safe and appropriate use of this.

Communicating with the wider MDT including GPs regarding any concerns about your patients.

Take an active role in team meetings, clinical audit, and service and team development.

Job responsibilities

Our neighbourhood team consists of physiotherapists and physiotherapy assistants, community nurses, occupational therapists, healthcare assistants working alongside the Worcester GPs to support people living in their own homes. This includes admission prevention and pathway one assessments as well as supporting urgent community response. The successful candidate will have the opportunity to work with a varied caseload which involves holistic falls assessment and treatment, orthopaedic, neurological, frail elderly, respiratory, musculoskeletal, palliative and end of life, urgent community need equipment, sudden deterioration, manual handling difficulties, and carer breakdown.
