Part-Time Advanced Nurse Practitioner: Triage & Primary Care
Job summary
A healthcare provider in Beaminster is seeking an experienced Advanced Nurse/Advanced Clinical Practitioner to join their friendly team. This position offers 20 hours per week, primarily on Mondays and Tuesdays. The successful candidate will triage patient requests and provide clinical care, while operating autonomously within their professional scope. Strong communication and clinical skills are essential. This role provides a supportive environment and aims to enhance patient care within the community.
Qualifications
- Must act autonomously within their professional scope of practice.
- Experience with triaging and managing patient caseloads.
- Ability to collaborate with multi-disciplinary teams.
Responsibilities
- Provide clinical care and triage for patients both face-to-face and via telephone.
- Coordinate care delivery and ensure involvement of patients and relatives.
- Monitor and improve health outcomes through effective management and screening.
