Practice Nurse

We are looking for a Practice Nurse who can show innovation and commitment to help us develop the services we offer to our patients within an ever-changing NHS environment.

Main duties of the job

The implementation of Health Promotion and disease management strategies (in line with the National Service Frameworks) and provision of a full range of nursing services to the patients of the practice.

Chronic Disease Management as required by the practice.

The completion of health checks, health screening, immunisation and vaccination programmes.

About us

The practice provides NHS services to 11,300 patients who live in Blaydon, Winlaton, Ryton and the surrounding areas. We work in a purpose-built 9‑year‑old building adjacent to the shopping centre.

The practice team consists of GPs, Practice Nurses, Health Care Assistants, Pharmacists, Administrators, Secretaries and receptionists as well as a team of attached staff from primary and secondary care.

Job responsibilities

JOB TITLE: PRACTICE NURSE

RESPONSIBLE TO: Clinically Lead Practice Nurse/GP Partners Non clinically Practice Manager

RESPONSIBLE FOR: Health Care Assistants

JOB PURPOSE: The implementation of Health Promotion and disease management strategies (in line with the National Service Frameworks) and provision of a full range of nursing services to the patients of the practice. Chronic Disease Management as required by the practice. The completion of health checks, health screening, immunisation and vaccination programmes.

Main duties and responsibilities
  • Undertake immunisations, injections and vaccinations.
  • Provision of health checks.
  • Carry out screenings, e.g. blood pressure, cervical smears, etc.
  • Carry out chronic disease reviews e.g. diabetic, hypertension, asthma, CHD.
  • Undertake ECGs.
  • Provision of travel advice.
  • Venepuncture and the taking and preparing of pathology samples.
  • Provision of general nursing duties, e.g. wound care and compression bandaging.
  • Providing lifestyle advice and information for patients.
  • Management of medical supplies stock, including stock control.
  • The inputting of information onto the Practice clinical system.
  • Ensure that infection control procedures are followed.
  • The ongoing cleaning and sterilisation and recording of clinical equipment and working surfaces.
  • Working within PGD and PDS as appropriate.
  • Participating in nurse, practice and wider team meetings.

Experience
  • Experience of working in accordance with the NMC Code of Professional Conduct.

Qualifications

  • Recognised qualification to diploma level or above.
