Our incredibly dedicated and experienced team work across one of the largest geographic populations across Mid Devon. With its blend of town and rural patients. Our patients benefit from the caring and compassionate delivery of care on a 1:1 basis, due to their housebound status, chronic diseases and terminal illnesses that prevent them from accessing healthcare in any other way. As a result, we are able to understand our patients needs in a more holistic way, embracing what is important to them and improving their experiences of healthcare.

Whilst working within patients' homes and care home providers, you will be supporting the delivery of a wide range of clinically acute and chronic interventions. This includes wound care management, venepuncture and long-term condition management as well as some end of life care and support. You will work as part of the wider multi-disciplinary team.

You will be need to hold a current driving licence to access the pool cars, as well as have your own access to an appropriate form of transport. As we strive to deliver a more person-centred care, promote independence and protect wellbeing, we help reduce unnecessary and often unwanted hospital admissions. This is especially important for our most rural communities and those who may find it difficult to access our services.

Although your working day will usually start and end at base, which is Tiverton Hospital, we do work flexibly to ensure that other areas are supported in times of need.

Working Pattern: 37.5 hours per week.

Pay: The salary advertised for this position reflects the Agenda for Change 2025/6 rates, inclusive of a 3.6% pay award which will be implemented later this year. If you join before the implementation date, the pay award will be backdated to your start date. Until then, remuneration will be based on the 2024/5 pay bandings (Pay scales for 2024/25 | NHS Employers)
