Overview

We are looking to recruit a full time Advanced Clinical Practitioner for Specialist Palliative Care.

This exciting new role will be part of the specialist palliative care (SPC) multidisciplinary team, working in partnership with the specialist palliative medical consultants, clinical nurse specialists, SPC physio, OT and therapy assistant. There will be regular liaison with the Hospice and Hospital SPC team. This role will clinically support the SPC caseload across 8 neighbourhood teams. Working within the four pillars of advanced practice we require a specialist palliative care ACP to support in shaping the future of palliative care in West Cheshire. Responsibilities

Main duties of the job

  • Provide team leadership in driving clinical workforce development and embed general palliative care competencies across community services.
  • Be a specialist palliative care advanced practitioner who is an experienced non-medical prescriber with advanced clinical expertise, knowledge, and an evidence-based approach within this specialism.
  • Develop enhanced clinical skills within the specialist team, facilitating clinical supervision, supporting autonomy and advanced clinical decision making.
  • Lead on the expansion of non-medical prescribing, empowering specialist palliative care clinicians and the wider workforce to deliver timely and effective symptom management.
  • Champion patient safety, continuous improvement and collaborative working with system partners.
  • Support driving quality, person-centred outcomes, research and innovation; focus on clinical audit, service evaluation and continuous quality improvement initiatives.

Qualifications – Essential
