We are looking to recruit a full time Advanced Clinical Practitioner for Specialist Palliative Care.

This exciting new role will be part of the specialist palliative care (SPC) multidisciplinary team, working in partnership with the specialist palliative medical consultants, clinical nurse specialists, SPC physio, OT and therapy assistant. There will be regular liaison with the Hospice and Hospital SPC team. This role will clinically support the SPC caseload across 8 neighbourhood teams. Working within the four pillars of advanced practice we require a specialist palliative care ACP to support in shaping the future of palliative care in West Cheshire. Main duties of the job

• Provide team leadership in driving clinical workforce development and embed general palliative care competencies across community services.
• Be a specialist palliative care advanced practitioner who is an experienced non-medical prescriber with advanced clinical expertise, knowledge, and an evidence-based approach within this specialism.
• Develop enhanced clinical skills within the specialist team, facilitating clinical supervision, supporting autonomy and advanced clinical decision making.
• Lead on the expansion of non-medical prescribing, empowering specialist palliative care clinicians and the wider workforce to deliver timely and effective symptom management.
• Champion patient safety, continuous improvement and collaborative working with system partners.
• Support driving quality, person-centred outcomes, research and innovation; focus on clinical audit, service evaluation and continuous quality improvement initiatives.

Qualifications – Essential

• Relevant Professional qualification - RGN
• Primary degree
• Master\'s degree in Advanced Practice, Palliative Care or equivalent
• Non-medical prescriber V300
• Clinical Examination and Diagnostics modules if not part of Master\'s qualifications
• Mentor qualification

Qualifications – Desirable

• Teaching qualification
• Clinical supervisor
• Community nursing specialist qualification

Essential

• Awareness of current national and local agenda in health and social care
• High level knowledge of multi-agency working (housing, social services, independent sector, etc)
• Knowledge and understanding of clinical governance
• Quality improvement and change management
• Ability to work in a highly demanding environment
• Teaching and mentorship in clinical settings
• Ability to work independently and make autonomous clinical decisions
• Knowledge of community profiling and health-needs assessment
• High level communication skills, both written and verbal

Essential

• Extensive post qualification experience in Specialist Palliative Care
• Portfolio of evidence showing achievements in practice/speciality
• Supervision experience and ability to support trainee advanced practitioners and practice nurses
• Ability to work autonomously at advanced practice level and make expert clinical decisions
• Experience leading related research and audit
• Communication of difficult messages to patients and families
• Negotiation and conflict management within multi-agency working
• Evidence of multi-agency/professional working
• Demonstrates leadership of teams in multiagency contexts

Desirable

• Advanced communication training
• Experience in patient involvement/patient feedback
• Experience in delivering nursing-led services
• Recent experience in primary and/or community settings
• Experience at an advanced clinical level
