A healthcare organization in Chester is seeking a full-time Advanced Clinical Practitioner for Specialist Palliative Care. The role involves providing team leadership, developing clinical skills, and championing patient safety. Candidates must hold a relevant professional qualification and possess extensive experience in Specialist Palliative Care. This position offers a salary between £55,690 and £62,682 annually.
We are looking to recruit a full time Advanced Clinical Practitioner for Specialist Palliative Care.
This exciting new role will be part of the specialist palliative care (SPC) multidisciplinary team, working in partnership with the specialist palliative medical consultants, clinical nurse specialists, SPC physio, OT and therapy assistant. There will be regular liaison with the Hospice and Hospital SPC team. This role will clinically support the SPC caseload across 8 neighbourhood teams. Working within the four pillars of advanced practice we require a specialist palliative care ACP to support in shaping the future of palliative care in West Cheshire. Main duties of the job
Cheshire and Wirral Partnership (CWP) provides health and care services for a population of over one million people, including mental health, learning disability, community physical health and all-age disability care, as well as three GP surgeries in Cheshire.
