Lead Clinical Pharmacist (Independent Prescriber)

We are seeking a skilled and experienced Lead Clinical Pharmacist with Independent Prescribing qualifications to provide senior clinical oversight and leadership to our pharmacy services. This role will focus on delivering expert clinical care, supporting best practice in medicines management, and developing the capability of the wider pharmacy team through training, supervision, and clinical audits.

This role focuses on clinical excellence, quality improvement, and building a culture of safe, evidence-based prescribing.

Main duties of the job

Clinical Care

  • Conduct structured medication reviews (SMRs) and manage patients with long-term conditions within your scope of practice
  • Prescribe, deprescribe and optimise medicines in line with clinical guidelines and best practice
  • Provide expert advice on prescribing and therapeutics to clinical colleagues
  • Review patients following hospital discharge or changes in medication
  • Support safe prescribing for care homes, vulnerable patients and those with polypharmacy

Clinical Leadership & Supervision

  • Provide clinical supervision and day-to-day support for other pharmacists and pharmacy technicians
  • Lead on case discussions, peer review, and learning from significant events
  • Support the induction and clinical development of newly appointed pharmacy staff
  • Identify training needs and help deliver clinical education sessions to the wider team

Quality Improvement & Clinical Governance

  • Lead on medicines audits and quality improvement initiatives across sites
  • Promote adherence to local formulary, NICE guidance, and medicines safety alerts
  • Support achievement of QOF, IIF, and PCN-related prescribing targets
  • Contribute to the development and review of prescribing protocols and pathways
