Priory Medical Centre is excited to welcome an enthusiastic and skilled Advanced Clinical Practitioner (ACP) to our progressive, patient-focused team in the heart of Warwick. This is a standout opportunity for an experienced ACP who is passionate about delivering high-quality, holistic care in a supportive and forward‑thinking primary care environment.

In this role, you will lead on the management of Long Term Conditions, with a particular focus on Diabetes, playing a crucial part in improving health outcomes for our diverse patient population. Alongside this, you will run same‑day acute triage clinics, using your advanced skills to assess, diagnose, treat, and prescribe for a wide range of presentations.

Main duties of the job

As an Advanced Clinical Practitioner at Priory Medical Centre, youll take on a varied and rewarding clinical role that allows you to work autonomously while being part of a supportive, high‑performing multidisciplinary team. Your core responsibilities will span both planned and urgent care, giving you the opportunity to make a real impact on patient outcomes every day.

Youll lead on the management of Long Term Conditions, with a focus on Diabetes, delivering comprehensive reviews, personalised care plans, medication optimisation, and proactive education to help patients better manage their health. Your expertise will support continuity of care and improved quality of life across our population.

Alongside this, youll run same‑day acute triage clinics, assessing, diagnosing, treating, prescribing, and referring for a wide range of presentations. Youll handle undifferentiated conditions with confidence, using your advanced skills to make safe, timely decisions and ensure patients receive the right care at the right time.

Youll also contribute to safeguarding, quality improvement, chronic disease management pathways, and collaborative MDT working. Your voice will help shape service development as primary care evolves.

This role is ideal for a motivated ACP who enjoys variety, autonomy, teamwork, and delivering outstanding patient‑centred care. Youll be valued, supported, and empowered to work at the top of your practice.

About us

Priory Medical Centre is a well‑established Warwick GP Partnership with a list size of approximately 18,900 patients and one branch surgery. We have 5 GP Partners and a full complement of additional staff including Salaried GPs, Advanced Nurse Practitioners, Practice Nurses, Health Care Assistants and administrative support. We are a recognised training practice for GP Registrars.

In June 2021, the practice moved into a brand new purpose built surgery designed with patient care in mind. The new building will be a modern, energy efficient & fit for purpose building that will enable the practice to provide better services to our patients. As a result of having the new facilities available, the practice closed the main Cape Road surgery, along with the Cape Road branch surgery. The new facilities will act as the main surgery, with a branch surgery still available to patients on Woodloes Park at the following address: 1 Brese Avenue, Woodloes Park, Warwick, CV34 5TS.

Job responsibilities

The following are the core responsibilities of the ACP in delivering health services.

a. Practice in compliance with their respective code of professional conduct and within their scope of practice, being responsible and accountable for their decisions, actions and omissions at this level of practice

b. Demonstrate a critical understanding of their broadened level of responsibility and autonomy and the limits of own competence and professional scope of practice, including when working with complexity, risk, uncertainty and incomplete information

c. Act on professional judgement about seeking help, demonstrating critical reflection on own practice, self‑awareness, emotional intelligence, and openness to change

d. Work in partnership with individuals, families and carers, using a range of assessment methods as appropriate

e. Develop, implement and embed health promotion and wellbeing programmes

f. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice

g. Implement and evaluate individual specialised treatment plans for chronic disease patients

h. Identify, manage and support patients at risk of developing long‑term conditions, preventing adverse effects to the patients health

i. Demonstrate effective communication skills, supporting people in making decisions, planning care or seeking to make a positive change

j. Use expertise and decision‑making skills to inform clinical reasoning

k. Initiate, evaluate and modify a range of interventions which may include prescribing medicines, therapies, lifestyle advice and care

l. Exercise professional judgement to manage risk

m. Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice

n. Be aware of duties and responsibilities regarding current legislation and adhere to practice policies and procedures on Safeguarding Adults and Safeguarding Children

o. Request and collect pathological specimens. Upon receipt, process and interpret pathology and other test results as required

p. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required

q. Maintain accurate clinical records in conjunction with extant legislation ensuring that SNOMED CT codes are used effectively

r. Maintain chronic disease registers and implement an effective call/recall system

s. Chaperone patients where necessary

t. Prioritise health issues and intervene appropriately

u. Support the team in dealing with clinical emergencies

v. Recognise, assess and refer patients presenting with mental health needs

w. Support in the provision of vaccination programmes for both adults and children

x. Be an extended and supplementary prescriber, adhering to extant guidance

y. Support patients in the use of their prescribed medicines or over the counter medicines (within own scope of practice), reviewing annually as required

z. Contribute to practice targets (QOF, etc.), complying with relevant guidance

aa. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)

bb. Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)

cc. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately

dd. Deliver opportunistic health promotion where appropriate

There may be, on occasion, a requirement to carry out other tasks, this will be dependent upon factors such as workload and staffing levels

In addition to the primary responsibilities, the ACP has the following wider responsibilities:

a. Provide holistic support in the clinical governance spectrum assuming a senior role in its compliance

b. Support the IPC lead for the practice, ensuring compliance

c. Monitor and ensure the safe storage, rotation and disposal of medications

d. Undertake overall management of the nursing team, providing guidance when necessary, acting as a mentor to students and newly qualified staff

e. Participate in local initiatives to enhance service delivery and patient care

f. Support and participate in shared learning within the organisation

g. Develop an area of specialist interest, taking the lead within the organisation

h. Continually review clinical practices, responding to national policies and initiatives

i. Drive the development of clinical services within the organisation, liaising with external agencies and professional organisations as required

j. Develop practice administrative and clinical protocols in line with the needs of the patient and current legislation

Qualifications

  • Proof of relevant professional and educational qualifications, coupled with details of any training

  • Conducting Triage Clinics for Minor Injuries / Minor Illness
