A healthcare organization in Amersham is seeking individuals with a friendly approach for a role supporting patients in their own homes. Key responsibilities include providing personal care and supporting the community to avoid hospital admissions. Candidates must have good interpersonal skills, and hold or be willing to pursue a Health and Social Care Level 2 or 3 Diploma. A full UK driving license is required.
Do you have a friendly 'can do' approach to life, and experience working as part of a health or social care team ?
We are looking for you to work as as part of the AmershamRapid Response & Intermediate Care team supporting patients in their own homes, providing personal, social and rehabilitative care. The overall aims of the Rapid Response & Intermediate Care Team are to avoid inappropriate admission of patients into Hospital and facilitate timely discharge into the community.
The team is committed to ongoing education on the job and development opportunities are available within the organisation. Successful applicants If who don\'t already have a Level 3 Qualification in Health and Social Care will be expected to work towards that in post.
