Job Title: Qualified Social Worker

Location: Barnsley

Rate: £35 per hour

An exciting role has recently become available for a qualified Social Worker. The Barnsley Metropolitan Borough Council is looking for a new member to join the Hospital Social Work team in Barnsley. This job is hybrid working but requires someone to respond to urgent requests to visit anywhere in the borough, combining home working, office-based, and hospital-based duties.

What will your responsibilities be?

• You must have strong social work values that guide your practice in helping people achieve the best outcomes.
• Be local to the area, as we are a reactive team, and be available at short notice to complete assessments.
• Apply knowledge of the Discharge to Assess Process to support individuals transitioning from hospital.
• Collaborate with health colleagues across acute and intermediate care settings.
• Complete Care Act and Mental Capacity Assessments, and lead on best interest decision-making.
• Conduct safeguarding investigations and contribute to complex discharge planning using a strengths-based approach.

Qualifications and Experiences:

To be successful in this role, you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have at least 12 months post-qualified Social Worker experience.
