An exciting new role has been developed within the innovative and welcoming discharge team, giving the successful candidate a unique opportunity to work with the wider MDT to enhance the experience patients have and improve efficiency of discharge and patient flow throughout the hospital.

• Attend daily board round and update / escalate to the discharge team on patients expected date of discharge (EDD), reason to reside status and/or any anticipated barriers to discharge.
• Advocate why not home? why not today?
• Work closely with Clinicians, Nursing teams and the wider multidisciplinary team (MDT) to support the proactive management of patients throughout the discharge process in a timely manner.
• Support patient flow team with cross‑cover to ensure safe operation and utilisation of the Transfer lounge on a rotational basis (aiming 1 day a month).
• Ensure key elements of the patients discharge pathway such as take‑home medication, District Nurse referrals, transport and home oxygen are requested in a timely way.
• Named ward allocation is anticipated, but cross‑cover for other inpatient wards will be required at times.
• Effectively communicate information to patients, relatives, carers and all members of the multidisciplinary team.
• Encourage the patient’s family and friends to be actively involved in the discharge process.
• Ensure that both electronic and paper documentation relating to a patient’s discharge are kept up to date and that key elements of a patient’s discharge process are recorded on the appropriate systems.

• Experience in oncology and discharge is desirable but not required.
• Ability to support patients with a planned timely discharge that is effectively managed and a positive experience.
