Job description

Dorset Healthcare are looking for a Community Rehabilitation Assistant to join our supportive Integrated Community Rehabilitation Team based at Purbeck. This is a 22.5‑hour a week post over 3 days, with weekends as part of a rota.

The multidisciplinary team works with adult patients in their own homes, community hospitals and care homes to promote independence and improve quality of life, supporting them to remain at home or be discharged home. Being a car driver is essential for this role.

You will join a motivated, caring, inclusive and compassionate team. You will work with nurses and therapists to set goals and assist patients to regain their independence. We also offer opportunities for further learning and development, including the chance to apply for nursing, physiotherapy and occupational therapy apprenticeships.

Responsibilities
  • Work mainly under the close supervision of clinicians within the team, but at times also autonomously.
  • Implement planned intermediate care/rehabilitation for individuals in their own home, community hospital ward and other community settings, maintaining a high standard of evidence‑based practice to meet identified needs.
  • Actively participate in learning opportunities to maintain knowledge and inform practice; seek feedback.
  • Manage own workload and time, e.g., maintaining own diary.
  • Assist clinicians in administrative tasks required for the efficient day‑to‑day running of the team.
  • Record own learning and personal development, identify and articulate areas for self‑improvement through self‑reflection and evaluation of practice.
  • Attend mandatory and statutory training as identified at appraisal and by trust policy.
Requirements
  • Enhanced Disclosure and Barring Service (DBS) check; the Trust will cover the cost.
  • Car driver licence – essential for the role.
  • Previous rehabilitation or care experience, working in the community or on a ward, desirable.
  • Ability to work within a community or hospital setting and support patients to regain independence.
