Vacancy at Cambridgeshire Community Services NHS Trust.

Cambridgeshire Community Services and Norfolk Community Health and Care will be merging to become one NHS Trust on 1st April 2026. We are committed to delivering high quality, patient centred care through collaboration, innovation and shared expertise, and expects all staff and volunteers to share this commitment.

We are seeking highly motivated Specialist Community Public Health Nurses to join our 0-19 Healthy Child Programme in Norfolk.

Key Responsibilities
  • The successful applicants will work as part of a dynamic team leading delivery of our Healthy Child Programme. This will include implementing planned programmes of care and evaluating care delivered.
  • Liaison with other agencies is also a key part of the role and will include consultations with professionals from other agencies.
  • The contacts are delivered face to face to support the range of issues experienced by children and young people aged 0-19 and their families.
  • To communicate complex and sensitive information concerning the child or young person’s condition to the child’s parents/carers and young person. Able to communicate effectively using appropriate methods and recognising barriers to communication.
  • To communicate within the multi-disciplinary team and model effective team working and communication skills, supporting change within the service.
  • To develop links with Early Childhood and Family Services (ECFS), educational settings and other agencies involved in promoting children’s well-being.
  • To work closely with other professional teams and key stakeholders including service users, statutory and non-statutory organisations.
  • To act as first line response to complaints working within national and corporate guidelines.
  • To contribute to the training of students of nursing and other disciplines, during community experience programmes.
  • To contribute to ensuring that CCS responsibilities to engage and consult with patients and the public are met in line with statutory requirements.
  • To demonstrate responsible use of resources, with an awareness of budgetary restraints.
  • To ensure health monitoring equipment is operating to a high standard, maintained and calibrated.
  • To maintain own professional development and requirement to take part in appraisal and KSF process.
  • To delegate work to other members of the team related to their capabilities and competencies, ensuring there is clarity regarding the feedback mechanisms.
  • To contribute to the development of local policies and procedures relevant to their area of work and contribute to relevant service design projects.
  • To take responsibility for their own clinical practice including clinical supervision and to adhere to the NMC code of conduct.
  • To support families through a range of methods including home visits, digital drop-in clinics, video contacts, to assess the needs of children and their families.
  • To support children and young people and their families with emotional and behavioural.
  • To advise their client group on the promotion of health and the prevention of illness.
  • To link practice with local and national targets and deliver public health programmes based on current priorities and targets.
  • To plan evidence based health promotion programmes, order resources, compile teaching plans with aims, objectives and outcomes, and evaluate the sessions.
  • To act as a health link for other agencies, e.g. Children’s Services, GPs, Midwifery Services both community and hospital, voluntary services, public and private agencies and work closely with partner agencies to promote the health and wellbeing of children, young people and families.
  • To work with the Locality Leadership Team to support children, young people and families who are considered to be vulnerable or at risk. To adhere to current Safeguarding Procedures, and participate in child protection case conferences and case review meetings as required.
  • To contribute to the development of local policies and procedures relevant to their area of work and contribute to relevant service design projects.
  • To take responsibility for their own clinical practice including clinical supervision and to adhere to the NMC code of conduct.
  • As service need requires, to support service delivery at the Single Point of Access in Just One Number.
  • To support other localities including the Single Point of Access to ensure that all relevant clinical skills are kept up to date, and to maintain effective working relationships and continuity of services.
  • Our organisation welcomes students to join our locality teams across Norfolk, to gain practice experience within a positive learning environment. Students are supported by Practice Supervisors and Practice Assessors, whose role is to support each student to progress in their learning journey, by providing practice examples to meet student’s learning needs.
  • To be competent and confident in using digital technology and be able to promote this to service users. This includes supporting the development of JustOneNorfolk.
  • To promote early intervention and the use of digital resources via Just One Number and Just One Norfolk to enhance self-care skills for children, young people, and their families.
  • To confidently utilise virtual platforms in assessments, interventions, training, and networking to support effective practice and partnership working.
  • Within this role you will need to travel as an independent practitioner on a daily basis, across Norfolk which involves both rural and urban travel. The nature of our work does result in travel arrangements changing at short notice. Due to the large geography our service covers and the limitations of sufficient public transport to undertake this role you must hold a full and current driving licence and have access to your own transport to meet the travel requirements of the role. This is subject to the provisions of the Equality Act 2010, and therefore if you require a reasonable adjustment due to a disability then this will be considered.
  • The contacts are delivered face to face to support the range of issues experienced by children and young people aged 0-19 and their families. To support the 0-19 service offer the post holder will be required to undertake migration training within 2 years of employment into the other SCPHN pathway, e.g. Health visitor to school nurse.
Qualifications and Requirements
  • Registered with the NMC and maintain professional registration.
  • Competent as a non-medical prescriber.
  • Adhere to infection control policies and procedures.
  • Adhere to the Trust risk assessment and risk management processes.
  • Undertake mandatory training and any other training relevant to the role as required by Cambridgeshire Community Services NHS Trust.
  • Participate in clinical and other audits as required.
  • Participate in clinical supervision on a regular basis.
  • Participate in relevant emergency preparedness process for their team.
  • Be competent and confident in using digital technology to promote self‑care and partnership working.
