A healthcare service provider in Ontario is seeking a Manager of Transitions Programs to lead operational management of transitional care. You will oversee interdisciplinary teams, ensuring quality client care and driving healthcare innovation. Ideal candidates will hold a relevant healthcare license and possess strong leadership and communication skills. Reporting to the Director Client Care, the Manager Transitions Programs ensures seamless, compassionate care as patients move from hospital to home. Designed to support both patients and their families, the program connects individuals with community-based healthcare providers, community support services, primary care and acute care teams. Together, they create personalized plans that improve outcomes and ease the transition to home.
The Manager, Transitions Programs provides operational leadership to transitional programs and the interdisciplinary teams that will be providing holistic care to the clients in the community. You are passionate about informing and shaping transitional programs from hospital to the community and being at the forefront of healthcare innovation in Ontario. You will provide exemplary leadership and operational management for interdisciplinary team(s) including nursing, therapies, social work, dietitian, PSWs and community support services care providers. You will ensure excellence in the provision of client care, the achievement of corporate / program objectives; planning and operations; quality management and improvement; participation in special projects, corporate / research initiatives and other activities that support and advance organization goals. This role will have accountabilities in operational and professional practice excellence; creating a healthy workplace that maximizes the potential of all members of the team.
