Job summary
A well-known non-profit organization is seeking a Transitional Care Assistant to facilitate client care from hospital to home. The role involves supporting clients in daily living activities, coordinating transportation, and working with care teams to ensure client's safety and health. Candidates should have a minimum of 3 years of experience in gerontology or social services, relevant educational qualifications, and strong communication skills. Proficiency in Microsoft Office is required. This full-time position involves working in various environments, including clients’ homes.
Qualifications
- Minimum of 3 years’ experience in a relevant field.
- Valid unrestricted driver's license required.
- Proficiency in Microsoft Office applications is mandatory.
Responsibilities
- Support discharge process and transportation home for clients.
- Complete errands to meet client’s physical needs.
- Work collaboratively within the circle of care.
