Responsibilities
- Provide direct nursing care in Home Health / Community Care - Primary Care and Home Services, and other units.
- Work with multidisciplinary teams to implement personalized patient care plans.
- Administer medications, monitor patients, and adjust care plans as needed.
- Educate patients and families on health management, disease prevention, and post-care plans.
