A day shift is 7.5hrs between the hours of 07:30 18.30, Mondayto Sunday including bank holidays). Working weekends will attract anenhancement payment however you will only be expected to work one weekend in afour week period.This is apart time post working 30 hours per week. Minimum of 22.5 hours.
The Worcester City Neighbourhood Team is a friendly, supportiveand hard-working team. We are lookingfor an experienced occupational therapist who is keen to deliver high qualitycare in the community setting. Worcestercity has two neighbourhood teams Riverside which is based in St Johns Housesurgery and Cityside which is based in Elbury Moor surgery. We also have a base at Warndon clinic whichis home to the admin support, shift leads and the therapy team.
The post holder will be integrated within the Riverside andtherapy team. You will be heavilyinvolved and instrumental in the journey of patients on the intensive supportpathway. This will involve assessingpatients who are being discharged from hospital as well as patients who requiresupport to prevent admission, identifying the appropriate intervention such asshort-term care support while the patient rehabilitates as well as anyimmediate equipment needs. The role isfast paced and responsive to urgent referrals and works closely with othernursing and therapy roles. Making a patients home safer and guiding thepromotion of independence is where the OT really adds value to the patientexperience.
Your typical day will include coordinating your caseload, triagingreferrals, allocating workload, supporting junior staff and assistants, andprioritising urgent response assessments alongside colleagues.
Alongside the intensive support pathway, you will be triaging andassessing patients who require functional occupational therapy assessments intheir own home. This involves manualhandling assessments and the provision of both specialised and standardequipment needs.
You will have opportunities for further training and developmentwithin the role. You will be joining an enthusiastic, supportive, and friendlyteam where you will receive support and development throughin-service training, regular appraisals, a personal development plan andclinical supervision.

The role will involve you:
Completing holistic assessments of patients needs followinghospital discharge or to prevent admission.
Referrals also come via single point of access from theneighbourhood team, Social Services andGPs. You will be completing assessmentsfor patients for low level equipment, adaptations and manual handlingneeds.
Initiate and co-ordinate individual rehabilitation programmes andsupport and educate assistants to deliver these effectively
Undertakingoccupational therapy assessments for a designated caseload, addressingoccupational performance and skill deficits, enabling the client in areas ofself-maintenance, productivity and leisure.
Assessingclients: in relation to their equipment and specialist equipment needs / forminor adaptations eg rails, half steps, and accora riser-recliner chairs.
Workingwith clients to identify Occupational Therapy goals as part of the overall careplan.
Monitoringand evaluating treatment in order to measure progress and ensure effectivenessof the intervention.
Act as key worker for patients on the intensive support team toco-ordinate their ongoing needs by liaising with the patient, GP, MDT,relatives and other agencies. This role will include organising individualclient case reviews, communicating plans with the MDT, and being responsible forthe ongoing actions in relation to the patient pathway.
Maintainingcontemporaneous clinical notes to the high professional standards of theCollege of Occupational Therapy and in accordance with Trust guidelines.
Linemanagement of band 5 and band 4 team members including training, supervisionsand appraisals.
