Job Summary: This position will ensure that the health and safety of all clients are met. The LPN will assist the Registered Nurse with planning, coordinating, and managing the care services and act within the state and federal regulations. The LPN is responsible for the overall direction, coordination, and monitoring of direct care service delivery under the direction of the Registered Nurse.

Compensation: $30-$33/HR- Paid as Salary

Responsibilities
  • Coordinate and oversee daily care activities, ensuring each resident receives personalized attention and support.
  • Collaborate with the Registered Nurse to develop and implement comprehensive care plans tailored to individual needs.
  • Monitor and document residents' health status, promptly addressing any changes or concerns with the healthcare team.
  • Lead and mentor a team of caregivers, fostering a supportive and collaborative environment.
  • Ensure compliance with state and federal regulations, maintaining the highest standards of care and safety.
  • Facilitate effective communication between residents, families, and healthcare providers to enhance care outcomes.
  • Participate in ongoing training and development opportunities to stay current with best practices in elder care.
Qualifications

Requirements:

  • Must be licensed with the State of Wisconsin or Minnesota as an LPN, and must maintain current unencumbered licensure.
  • Previous experience in a related field.
  • Maintains current background check on file with the State of Minnesota or Wisconsin and agencies involved.
  • Must have a valid Minnesota or Wisconsin Driver’s License.
  • Drives company and personal vehicle in a safe manner and in accordance with MN and WI State Laws.
  • Must be able to read, write, and communicate in English.
  • Attention to detail and well-organized
  • Flexibility and willingness to help wherever needed
