STATEMENT OF PURPOSE
To achieve a high standard of customer service, support and coordinate all the clinical care for renal patients in the inpatient and outpatient setting.
MAJOR DUTIES AND RESPONSIBILITIES
SPECIFIC (80%)
Patient Care
- Case management for early chronic kidney disease (CKD) patients to delay their disease progression through education on condition, follow up with appointment, evaluate their investigation results and discuss with primary physician on the appropriate intervention.
- Case management for advanced CKD patients & their family members for smooth transition from non-dialysis dependent to dialysis dependent through pre-dialysis counselling, coordination of access creation, follow up patient’s decision on modality, application/approval of Voluntary Welfare Organisation (VWO) and interim haemodialysis (HD) centre arrangement.
- To liaise with vendors, dialysis centres, patients and caregivers pertaining to end stage kidney disease.
- To conduct Peritoneal Dialysis (PD) competency for patients deciding for PD.
- To conduct Advance Care Plan (ACP) discussion for pre and post initiation dialysis patients in the inpatient and outpatient setting.
- To follow up patient’s care including hepatitis B vaccination, anaemia management, calcium phosphate control and nutrition status.
- To monitor and track statistics on the workload and quality outcome.
- To track patients with dialysis related issues such as adherence to treatment, cardiac related complications, anaemia and nutrition especially patients who are under renal transplant registry and wait list.
- To coordinate the care of patients who are under renal transplant wait list and need yearly review at transplant centre in the tertiary hospital.
- To monitor the outcome of inpatient dialysis related complications and hospitalisations.
- To work with nephrologists, surgeons, dialysis nurses, medical social workers, dietitians and pharmacist to ensure quality patient care.
- To arrange and coordinate seminar/workshop on kidney disease.
- To complete the monthly report, track the workload, key performance indicators and balance score card.
- To file and handle all patients’ documents systematically, orderly and confidentially to prepare for respective audits when required.
- To submit monthly HD and PD patient statistics to National Registry Disease Office (NRDO).
GENERAL (20%)
- Initiate self-growth and development through on the job training.
- To ensure that confidential information is safety kept at all times.
- To identify problem area, recommend and implement improvements.
- Administrative and data collection.
- Perform any other ancillary duties as assigned by Nephrologist/DOA/HOD/COO.
- Embrace continuous improvements as our way of life by improving and refining work performance on a daily basis.
JOB REQUIREMENTS
EDUCATION, TRAINING AND EXPERIENCE
• Bachelor’s Degree in Nursing.
• 2 years working experience, preferably with renal related experience.
• Basic IT knowledge is required. Good PC knowledge is an advantage.
• Possess positive attitude and consistent good performance.
• Possess good interpersonal and communication skills.
• Possess positive attitude, initiative and keenness to learn.
• Able to work independently.
• Be proactive and able to implement the change.
PROFESSIONAL LICENCE
• Singapore Nursing Board (SNB) licence.
