Job description
Summary

Coordinates with nursing, medical, and manager, care coordination for the assessment, planning, implementation, and evaluation of care to specific patient populations. Contributes to the achievement of Care Coordination strategic goals.

Essential Responsibilities and Duties
  1. Contributes to optimal bed utilization by ensuring an appropriate plan of care corresponding to the planned discharge time frame.
  2. Demonstrates collaborative multi-disciplinary partnerships working towards the achievements of constantimprovements in quality patient care.
  3. Contributes to the achievement of clinical quality outcome indicators and patient satisfaction.
  4. Develops and implements policies and procedures related to Care Coordination and discharge planning in collaboration with the multidisciplinary team incorporating evidence-based practice as required and assigned.
  5. Coordinates and documents patient care utilizing a Care Coordination plan, medical records/ICIS, and patient teaching. Documents all relevant information correctly, appropriately, and in a timely manner.
  6. Maintains records and data collection in area of responsibility. Develops statistical and graphical reports indicating development and advancement toward Care Coordination goals and objectives.
  7. Liaises with internal and external organizations in the exchange of information, benchmarking, and in the coordination of activities relating to best practices in Care Coordination.
  8. Contributes to decreasing the length of stay/waiting time as per the section-specific data.
  9. Promotes a safe environment and safe patient care by embracing a safety culture and identifying areas of risk to the Manager, Care coordination using Root Cause Analysis (RCA). Complies with policies for personal and patient safety and for the prevention of healthcare-associated infections.
  10. Contributes to the achievement of cost efficiency savings in assigned areas. Maintains an awareness of resource utilization, exercising care in the ordering and use of equipment and materials.
Education

Bachelor's Degree in Nursing or other Clinical discipline is required.

Experience Required

Four (4) years of Nursing/Clinical experience is required.

Other Requirements (Certificates)

Saudi Commission for Health Specialties Licensure is required.
