A regional healthcare provider in Uttlesford seeks a qualified nurse to provide comprehensive nursing care in the community. The role involves patient assessments, developing care plans, and collaborating with healthcare teams. Ideal candidates should have a nursing qualification and experience in community settings. The post holder will be responsible for assessment of care needs, development, implementation and evaluation of programmes of care for patients within the Team Leader’s sphere of care. The post holder works without direct supervision and is responsible for nursing care for the geographical area including weekends and evenings. The community team of West Essex is part of the wider Hertfordshire and West Essex Integrated Care System supporting Out of Hospital Care to support West Essex Residents. The post holder will be working as part of the Integrated Neighbourhood Teams supporting the North Uttlesford Integrated Neighbourhood Team with a focus on prevention of admissions and facilitating hospital discharges.
