To work with the Multi-disciplinary Team to promote an efficient, effective service for patients and carers.
Work as an autonomous practitioner, both professionally and legally accountable for all aspects of the professional and clinical work.
Focus therapeutic intervention for symptoms and difficulties associated with presenting diagnosis; being guided by recommendations from up-to-date NICE Guidelines.
To have responsibility for the therapeutic management of patients within the speciality care pathway.
To be responsible for a designated caseload of patients, where you will assess patients, implement and evaluate treatment plans, and coordinate therapeutic interventions.
You will assist in the organisation, development, promotion, and delivery of the Service.
To liaise with health, education, and social services, working in partnership with primary and secondary care, social prescribers, and other external agencies to ensure the delivery of high-quality care.
Make recommendations to the team leader for changes to practice, lead the implementation of specific changes as required, and contribute to service guidelines.
