A leading healthcare provider in Liverpool is seeking a qualified professional to join the ICRAS model, coordinating care and driving quality improvement. The ideal candidate will hold a degree and relevant professional registration, and have a strong background in management and community care. Responsibilities include ensuring responsive services to local needs and engaging with multidisciplinary teams to promote integrated health and social care.
The post holder will work as part of a dynamic multi‑agency integrated team assessing referrals regarding ICRAS Service requirements for patients across Liverpool and South Sefton.
The post holder will —
• Ensure due regard to patient confidentiality, interpret and share necessary information to promote ICRAS across organisations. • Be responsible for co‑ordinating care delivered through the ICRAS model. • Support the delivery of the ICRAS model and influence service development. • Operate within complex health and social care systems, including Health and Social Care and third sector providers. • Provide leadership within ICRAS services. • Teach and present to groups of staff, changes in both ICRAS Service provision and direction and ensure change in policy is implemented across the whole system. • Coordinate interventions for patients, communicating with GP practices, Secondary Care providers and collaborating with staff inside and outside the ICRAS model. • Take ownership of the ICRAS model for continuous quality improvement, innovation, and integration. • Contribute to the development and take responsibility for the delivery of the quality strategy within the designated area, incorporating the establishment of systems and processes.
• Ensure smooth running of respective ICRAS teams. • Clinically review, coordinate and distribute referrals, directing staff accordingly. • Manage MDT meetings/SAFER huddles including preparation and facilitation. • Coordinate health professionals outside core specialist services for input into patient care or MDT attendance. • Manage incidents and complaints. • Ensure the interface between acute hospitals, primary care, community health services, social care, housing, the independent sector, and voluntary sector is robust at all times. • Maximise the capacity of ICRAS teams. • Ensure ICRAS services are responsive to local population needs. • Engage with other health and social care systems around good practice for ICRAS and research models of service. • Ensure ICRAS Care providers deliver high‑quality care appropriate to client needs. • Ensure effective use of Integrated Care services across the Whole System. • Support analysts in reporting data, identifying information gaps and pressures within the whole ICRAS system. • Ensure standards of service are delivered in line with local and national guidelines across all professional groups. • Support line management and act as authorised signatory for identified accountable staff. • Support training and development of ICRAS staff.

Professional — Maintain current professional registration. Act within the boundaries of the Code and Scope of Professional Conduct. Maintain contemporary records as per policies and NMC guidance. Be involved in Clinical Supervision and act as a reflective practitioner. Maintain personal professional development by completing requirements of PREP, actively participating in Personal Development Reviews, completing mandatory training and attending staff meetings. Provide evidence‑based practice and a high‑quality integrated care service tailored to individuals within the ICRAS.
Management — Maintain constructive relationships with a broad range of stakeholders. Coordinate complex activities across NHS and non‑NHS organisations. Record, interpret, and present health information impacting patient risk or needs. Develop and implement data collection systems and ICRAS protocols. Disseminate and communicate decisions and health information to relevant professionals. Identify service gaps and issues relating to effective clinical pathways. Escalate evidence of ineffective arrangements. Participate in internal and external working groups and projects. Carry out any other duties as may be reasonably required by line management.
Leadership — Contribute to performance improvement of the ICRAS. Coordinate and participate in relevant working groups. Provide advice and support to staff. Report on the function of the ICRAS to service lines. Investigate variance in activity and contribute to solutions. Identify risks and issues to facilitate resolution or escalation.
Clinical — Act as a resource on ICRAS care for other disciplines, agencies and the public. Prioritise and manage workload effectively. Support team members’ professional development. Provide cover for absent care coordinators. Follow all policies and procedures. Provide clinical leadership as appropriate.
Non‑Clinical — Provide regular reporting and monitoring from a quality and safety perspective. Contribute to strategic planning of ICRAS team projects. Develop performance and governance strategies. Contribute to short, medium and long‑term service developments. Manage workload and make informed decisions. Use software for reports. Contribute to the drawing up of the ICRAS team budget. Hold a designated budget and purchase physical assets and supplies.
Training & Education — Participate in annual performance reviews and continuous professional development. Take a lead in coordinating and providing training and development to ensure a wide understanding of the ICRAS. Provide internal and external training development of the ICRAS team. Practice in accordance with professional, ethical and legal codes and Mersey Care NHS Foundation Trust protocols and guidelines. Be accountable for own practice.
