Adult Community Team North is a large and supportive team consisting of Social Workers, Occupational Therapists, and Social Care Practitioners covering areas in North Wiltshire.

We work with adults over the age of 18 in need, actively listening, collaborating, and tailoring plans to enhance well-being. We partner with healthcare providers to prevent crises, emphasizing existing support for independence. Our approach seeks local, sustainable solutions and offers diverse intervention and prevention opportunities.

Registration with Social Work England is a must. Strong interpersonal skills and effective communication are essential. An updated knowledge of relevant legislation, safeguarding vulnerable adults, and an in-depth understanding of Health and Social Care legislation and policy are vital.

To uphold standards of practice for social workers as defined by the Health and Care Professions Council and in accordance with the Professional Capabilities Framework for Social Workers. Practitioners working at the Social Worker level are expected to practice effectively, exercising higher quality judgments in situations of increasing complexity, risk, uncertainty, and challenge. They anticipate issues that may develop, exercise greater independence, and use their initiative to broaden their responses. They have expertise in one or more areas of practice, are familiar with local resource networks, and are recognized as reliable sources of knowledge and advice. Specific duties include:

• Undertaking person-centered social work with customers and planning support/services to help them meet their outcomes and live as independently as possible.
• Ensuring accurate and timely recording of all information and activity onto the social care records database, in line with work targets.
• Supporting carers to continue in their roles.
• Having a well-developed understanding of assessment, support planning, and risk management, applying Council policies, and supporting customers to manage risks appropriately.
• Gathering sufficient information to accurately assess needs against eligibility criteria and signposting/advocating accordingly.
• Following Council policies and procedures at all times.
• Carrying out timely, person-centered support plan reviews and evidencing the need for ongoing support.
• Contributing proactively to team effectiveness through knowledge sharing and support.
• Engaging fully in supervision and professional development.
• Representing the Council professionally at meetings with customers and external partners.
• Attending Civil Emergencies when required.
• Occasional flexible working outside standard hours, not exceeding 10% of contracted hours per week.

• Qualified Social Worker
• Social Work England registration
• Up-to-date knowledge of strengths-based social work with adults, including safeguarding
• Understanding of health & social care legislation and policies
• Effective communication skills
• Proficiency with Microsoft Office and social care record systems
• Organized, customer-focused, and self-motivated
• Ability to work collaboratively and produce clear reports
• Relevant professional and personal development experience
• Fluent in spoken and written English
• Post-qualification experience in adult services

• Experience of multi-disciplinary working and partnership collaboration
• Assessment and investigation skills
• Ability to work with challenging client groups
• Excellent interpersonal, communication, and presentation skills
