A healthcare provider in Greater London seeks a care coordinator to assess and treat mental health issues in children and young people. You will conduct risk assessments and manage referrals while collaborating with a multidisciplinary team. This role requires effective management of complex cases where 75% of your time will be spent in clinical practice. The post holder will be a care coordinator responsible for assessing and treating a range of mental health and significant behavioural difficulties in children and young people who are referred to the team.
You will carry out risk assessments with children & young people and their families routinely and work under the supervision of the team manager and senior clinical supervisor to manage these situations effectively.
You will be part of the daily duty rota for the service, which involves screening and managing new referrals, telephone queries and supporting the team where necessary.
You will work with other CAMHS practitioners in the service from all disciplines. You will be a member of a number of internal and external professional networks to ensure that care plans for individual children and services across agencies are co‑ordinated efficiently and effectively. This post is a clinical post and the expectation is that 75% of your time is spent in clinical practice.
The Team hold smaller caseloads due to high levels of risk and complexity, there is supervision and MDT to help manage the work.
This role is for the Adolescent Treatment Team, a specialist team within Lewisham CAMHS, who provide stabilisation, formulation treatment and discharge to high‑risk complex mental health presentations for children in Lewisham.
The successful candidate will be a care coordinator responsible for assessing and treating a range of mental health and significant behavioural difficulties in children and young people who are referred to the team. The successful candidate would also take part in managing the physical health clinic with other nurses.
They will carry out risk assessments with children & young people and their families routinely and work under the supervision of the team manager and senior clinical supervisor to manage these situations effectively. They will also be part of the daily duty rota for the service, which involves screening and managing new referrals, telephone queries and supporting the team where necessary.
If successful you will work with other CAMHS practitioners in the service from all disciplines. You will be a member of several internal and external professional networks to ensure that care plans for individual children and services across agencies are co‑ordinated efficiently and effectively. This post is mostly client facing to manage the high risk and complex presentations of the young people.
