A healthcare recruitment company is seeking a Practice Nurse in London. The successful candidate will provide evidence-based nursing care and manage patients with long-term conditions. Our client is a dynamic, multi-award-winning teaching Partnership, serving patients in the community for more than 30 years, in the Woodberry Wetlands region looking after circa 12,000 patients.
The post holder is responsible for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long-term condition, and the provision of preventative nursing activity to the practice population. As autonomous practitioners the nurse is responsible for the care they deliver, demonstrating critical thinking and skills in clinical decision making in the management of patients. They will work collaboratively within the practice team to meet the needs of patients, supporting the delivery of policy and procedures and providing nurse leadership as required.
