Part-time Performer Dentist required for 2-3 days per week in a MODERN, INDEPENDENTLY OWNED Dental and Orthodontic practice in Wolverhampton, West Midlands.

1. £15 PER UDA
2. 3200 UDA target

4. Principal is a Specialist Orthodontist
5. Orthodontic mentoring by Specialist Orthodontist if desired
6. 50% split on Private and Labs
7. Modern, fully computerised surgeries - 3 surgery practice
8. iTero scanner
9. OPG/CEPH
10. Digital x-rays
11. Rotary endo
12. Longstanding support team
13. FREE on-site parking
14. 40 minutes from Birmingham, 35 minutes from Telford and 30 minutes from Stourbridge
15. Available now, start date negotiable

Compliance requirements:

• Full NHS Performer Number
• Local Area Team confirmation
• GDC certificate
• UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years
• Indemnity certificate
• Passport
• Enhanced DBS (within the last 12-36 months)
• Clinical references covering places of employment over past 2 years (minimum 2 referees)
• Degree certificate
• CPR/AED/Medical Emergencies certificate dated within the last 12 months
• IRMER
• PIN number
