The Neighbourhood of Wellingborough, consisting of 8 GPPractices are seeking to develop an innovative new model of care.

You will be working with a team of GPs and other specialistANPs to deliver same day care to the Wellingborough population.

Role Summary

As a Nurse Practitioner you will provide autonomous care for our patients presenting with minor/acute illness, and common general practice presentations including those relating to womens health. You will demonstrate safe clinical decision making and provide expert care by undertaking initial history taking, clinical assessment, diagnosis, treatment and evaluation of care.

Overview of Responsibilities

Clinical

• Provide clinical care to our patients. This will include reducing the demand on the GP time by the assessment and manage of patients and in the community by means of telephone triage and clinic appointments.
• Use advanced skills and expert knowledge to assess the physical and psycho social needs of patients with common primary care conditions and minor illness, instigating and providing in partnership with other care providers, therapeutic treatments based on best available evidence in order to improve health outcomes.
• Use advanced skills and expert knowledge to make comprehensive and focussed assessments, independently and proactively managing care of the patients.
• Promote patient independence and self-care where appropriate.
• Undertake the role of independent non-medical prescriber in the treatment and care of patients
• Review pathology results to ensure appropriate treatment and care
• Provide appropriate non clinical support to the GPs as per the practice needs.
• Support and lead the practice for the delivery of the Quality and Outcomes Framework and NHS England/Nene CCG/Federation Contracts.

Regulatory

• Respond to complaints and clinical incidents/significant events as appropriate to ensure robust systems are in place for improving practice through joint learning.
• Undertake at least one clinical audit a year. Evaluate data and present findings to the Practice Clinical team. Implement changes where appropriate.
• Adhere to the Practice Policies and Procedures including Infection Control, Health and Safety
• All staff must be familiar with, actively promote and work within the spirit of the practices Equal Opportunities Policy at all times.
• All staff who have access to personal data in relation to patients or staff will be aware of their responsibilities under the Data Protection Act 1998 and will abide by the eight principles of that Act.Any breach of the Act could result in disciplinary action being taken and criminal charges being brought against the individual.

Personal / Professional Development

• Maintain own personal / professional development plan and portfolio thereby meeting the requirements for revalidation through appraisal of own performance and discuss development needs with mentor, supervisors and managers, on a regular basis.
• Maintain NMC registration and professional development in line with expectations of the role
• Work at all times within the NMC Code of Professional Conduct.
• Provide Mentorship and training of new recruits or students as and when required.
• Participate in the Practice Performance Review process.

• RGN with a minimum of 5 years post registration experience
• Independent non-medical prescriber
• Community/practice nurse qualification or equivalent experience
• Extensive clinical experience in primary nursing

• Used to prioritising varied workloads to meet deadlines
• Used to dealing with all levels of staff
• Evidence of ability to teach/disseminate knowledge and skills
• Post registration qualification in relevant areas such as minor illness/injury, womens health/mental health/paediatrics
• Teaching qualification
• Recent attendance on a leadership skills development programme

• In depth knowledge base in the management of patients with chronic illness and minor illness
• Ability to maintain and monitor high standards of evidence based care
• Ability to direct and co-ordinate care working autonomously and collaboratively
• Understanding of clinical governance issues
• Evidence of improving the patients experience
• Evidence of leading a service project at an practice level
• Undertaking research or involvement in clinical research programmes
• Experience in writing clinical policies and protocols
• Counselling experience

• Excellent communication (verbal, written, and IT)
