Job description
Vaccination and Immunisation Care Coordinator

The post ispart of the Affinity Care Primary Care Network Team, and will provide day to daysupport in the bookings of appointments for patients linked to the roll out ofour vaccinations (vaccs) and immunisation (imms) programmes, including the plannedCOVID Vaccination programme, the influenza programme, Shingles, baby vaccs andimms, other adult immunisation programmes such as HPV, Pertussis in pregnancy,Meningitis ACWY and any catch-up programmes as introduced from time to time.

Main duties of the job

Handle enquiries from the general public aroundscheduled vaccination programmes including but not limited seasonal influenza,Covid vaccinations and child immunisations.

Proactively follow up with patients to book intheir vaccinations in line with eligibility criteria as defined by the NHS.

Undertake any tasks consistent with the level ofthe post and the scope of the role, ensuring that work is delivered in a timelyand effective manner.

Job responsibilities

Understand the eligibility andrequirements for all vaccines, immunisations and screening.

Support the development ofsearches within SystmOne to clearly identify eligible cohorts and provideaccurate data relating to the various programmes

Use SystmOne searches toidentify patients eligible for the vaccines, immunisations and screening tosupport workload planning and booking activity

Co-ordinate the booking ofappointments for these patients into appropriate clinics within agreed timescales

Co-ordinate the planning andbooking of care home and housebound patients in partnership with the ComplexCare Team (CCT)

Support the PSM and Nurse Leadto ensure all vaccines are ordered pro-actively to appropriate levels and stockreturns submitted using agreed systems

Support the PSM and Nurse Leadto ensure the distribution of vaccines across all sites as relevant to plannedactivity

Work with the PSMs and Care-Cordinators to provide rotas forboth the booking team and vaccination / immunisation / screening teams

Support the administrative andvaccination teams to provide a quality system of work that is effective in itsuse of team resources

Apply proven approaches toencourage attendance and reduce DNAs

Constantly seek serviceimprovement, mindful of the patient experience and patient safety balanced withefficient use of our resources

Maximise the use of newtechnology and methods of communication withpatients

See things from a patientperspective; identifying ways to improve our systems to enhance the patientexperience

Contact eligible patients,provide supporting information as necessary to patients and book appointmentsas appropriate

Capture such knowledge in a waythat it can be shared with other care coordinators and inform other teamswithin Affinity Care

Undertake any tasks consistentwith the level of the post and the scope of the role, ensuring that work isdelivered in a timely and effective manner.

Escalate all relevant issuesand information to their Line Manager, ensuring they are aware of anydevelopments impacting the operational functioning of the service

Support the collation of allactivity data and reporting information as required

Person Specification
Qualifications
  • GCSE English Language, Grade C or above (or equivalent)
  • NVQ Level 3 or equivalent level of knowledge in office procedures and systems or customer service
Experience
  • Evidence of experience in wide range of administrative systems and software programmes
  • Experience of direct patient or customer communication i.e. face to face or via telephone
  • Familiarity with the names of vaccines and immunisations
  • Experience of and competency in using the SystmOne clinical system
  • Evidence of ability to support collation and analysis of data
  • Understanding of Data Protection legislation
  • Experience of supporting and developing service improvement
