Overview

The Team Lead Palliative Care Clinical Nurse Specialist (CNS) will manage and lead the palliative care CNS team at West Middlesex University Hospital. The post holder will deliver expert evidence‑based care for patients with complex specialist palliative care needs and support the multidisciplinary team in caring for those with a life‑limiting disease.

Responsibilities
  • Provide expert, evidence‑based care and management of patients with complex specialist palliative care needs.
  • Deliver specialist advice and support to the CNS team and all members of the wider multi‑professional team.
  • Collaborate with Primary Care colleagues, hospices, community palliative care teams, social services, care homes, community matrons, district nursing teams and voluntary organisations to provide a seamless integrated pathway.
  • Participate in the seven‑day face‑to‑face service and manage staffing to facilitate this.
  • Assist with service improvement, research, audit and team management duties within a weekly timetable.
  • Encourage and facilitate team participation in service improvement, research and audit.
  • Act as an exemplar of professionalism, motivating and influencing others to improve and achieve their potential.
Qualifications
  • Minimum three to four years’ experience as a Band 7 Palliative Care CNS (two of these years must have been in an acute hospital setting).
  • Strong knowledge, skills, behaviours and nursing proficiencies required to work effectively as a team.
  • Experience of leading teams in a dynamic, developing service environment.
