Job Summary:
The Registered Nurse with advanced and specialized knowledge and clinical skills in wound, ostomy, and continence care who has graduated from a recognized education program is responsible for providing and managing client and family-centered, evidence-based, efficient nursing care to individuals and families in their homes and communities. The Registered Nurse ensures care is provided within the framework of quality & risk management and the standards and scope of practice set by VON and relevant provincial regulatory bodies.
Key Responsibilities:
- Performs detailed holistic assessments of clients with wound, ostomy, and/or continence concerns.
- Formulates treatment plans in collaboration with the interprofessional team based on holistic assessments.
- Provides education and mentorship to build wound, ostomy, and continence knowledge within VON.
- Provides consultation to care providers within the organization.
- Collaborates with the interprofessional team across the continuum of care.
- Shares clinical knowledge to enable clients, families, and communities to make informed health decisions.
- Remains updated on current evidence related to best practices in wound, ostomy, and continence care.
- Ensures services are consistent with standards established by VON and provincial regulatory bodies.
- Documents nursing processes clearly, accurately, and appropriately, according to standards.
- Documents client and family responses to care interventions.
- Promotes client and family involvement in health and service decisions.
- Negotiates care delivery with clients or families to respect their choices.
- Refers clients to appropriate nursing providers and other disciplines based on established guidelines.
- Identifies and utilizes tools and resources efficiently.
- Promotes client-directed care, self-care, and independence through teaching and health promotion.
- Uses adult learning principles and established materials for health education.
- Influences policy review and development through involvement in professional and community organizations.
- Practices in accordance with quality and risk management frameworks and Occupational Health and Safety legislation.
- Participates in creating and following up on client care plans to promote quality improvement.
- Performs other duties as assigned.

Education, Designations, and Experience:
- Must be licensed as a Registered Nurse in the province of practice.
- Completion of NSWOCC WOC-EP program, or IIWCC, or hold a Master’s as an Advanced Practice Nurse with formal skin and wound care education.
- Three years of community or acute care wound care experience preferred.
- Experience in community and home care preferred.

Working Conditions and Physical Demands:
This role requires attention to detail in a dynamic environment, involving physical activities such as lifting, carrying, bending, reaching, kneeling, and other movements emphasizing good body mechanics. Tasks include walking, sitting, standing, climbing stairs, and fine hand movements.
