Program: Visiting Nursing

On-Call: Yes | Hours bi-weekly: 60

Job Summary:

The Registered Nurse with advanced and specialized knowledge and clinical skills in wound, ostomy, and continence care who has graduated from a recognized education program is responsible for providing and managing client and family-centered, evidence-based, efficient nursing care to individuals and families in their homes and communities. The Registered Nurse ensures care is provided within the framework of quality & risk management and the standards and scope of practice set by VON and relevant provincial regulatory bodies.

Key Responsibilities:
  1. Performs detailed holistic assessments of clients with wound, ostomy, and/or continence concerns.
  2. Formulates treatment plans in collaboration with the interprofessional team based on holistic assessments.
  3. Provides education and mentorship to build wound, ostomy, and continence knowledge within VON.
  4. Provides consultation to care providers within the organization.
  5. Collaborates with the interprofessional team across the continuum of care.
  6. Shares clinical knowledge to enable clients, families, and communities to make informed health decisions.
  7. Remains updated on current evidence related to best practices in wound, ostomy, and continence care.
  8. Ensures services are consistent with standards established by VON and provincial regulatory bodies.
  9. Documents nursing processes clearly, accurately, and appropriately, according to standards.
  10. Documents client and family responses to care interventions.
  11. Promotes client and family involvement in health and service decisions.
  12. Negotiates care delivery with clients or families to respect their choices.
  13. Refers clients to appropriate nursing providers and other disciplines based on established guidelines.
  14. Identifies and utilizes tools and resources efficiently.
  15. Promotes client-directed care, self-care, and independence through teaching and health promotion.
  16. Uses adult learning principles and established materials for health education.
  17. Influences policy review and development through involvement in professional and community organizations.
  18. Practices in accordance with quality and risk management frameworks and Occupational Health and Safety legislation.
  19. Participates in creating and following up on client care plans to promote quality improvement.
  20. Performs other duties as assigned.
Common Responsibilities:
  • Promotes the goals and values of VON as an integrated community care provider.
  • Ensures a safe and healthy workplace in compliance with OHS legislation and VON policies.
  • Maintains confidentiality and protects personal information.
  • Works collaboratively within a team approach to service delivery.
External and Internal Relationships:
  • Engages regularly with clients, families, and caregivers, providing education and support for independence.
  • Participates in community and professional organizations to promote nursing best practices.
Education, Designations, and Experience:
  • Must be licensed as a Registered Nurse in the province of practice.
  • Completion of NSWOCC WOC-EP program, or IIWCC, or hold a Master’s as an Advanced Practice Nurse with formal skin and wound care education.
  • Three years of community or acute care wound care experience preferred.
  • Experience in community and home care preferred.

Other Requirements:
  • Current satisfactory Criminal Records Check.
  • Owns a personal vehicle with valid driver’s license and insurance.
  • May require Vulnerable Sector Search and/or Child Abuse Registry Check.
  • Availability for flexible hours.
Working Conditions and Physical Demands:

This role requires attention to detail in a dynamic environment, involving physical activities such as lifting, carrying, bending, reaching, kneeling, and other movements emphasizing good body mechanics. Tasks include walking, sitting, standing, climbing stairs, and fine hand movements.
