Our Clinic nurses provide care in VON community clinics, including client assessments, wound care, IV medications, and education. Visiting nurses provide care in clients' homes, including assessments, symptom management, palliative care, rehabilitation, wound care, education, and counseling.

The Registered/Licensed Practical Nurse is responsible for delivering evidence-based, client and family-centered nursing care in community and home settings, ensuring safety, quality, and adherence to regulatory standards. The role includes teaching, delegation, and collaboration with healthcare teams.

1. Provide nursing care to clients with less complex needs according to established care plans, consulting with RNs for more complex cases.
2. Manage and evaluate care via nursing care plans, responding to client outcomes.
3. Conduct comprehensive assessments of clients and families, documenting findings clearly and accurately.
4. Develop and modify care plans in collaboration with the healthcare team, justifying changes.
5. Educate clients and families to promote independence and self-care, using adult learning principles.
6. Refer clients to appropriate care providers and resources, ensuring continuity of care.
7. Participate in quality improvement initiatives, policy development, and professional standards.
8. Maintain competence through ongoing education and self-reflection.

• Collaborate within multidisciplinary teams, acting as a resource and mentor.
• Engage with clients, families, and caregivers to support health and independence.
• Contribute to community and professional organizations to promote best practices.

• Current license as a Registered or Licensed Practical Nurse in the province of practice.
• Clinical practice experience is preferred; community health experience is an asset.

• Effective assessment, communication, and interpersonal skills.
• Ability to adapt nursing interventions and problem-solve.
• Knowledge of quality and risk management principles.
• Strong organizational and time management skills.

• Criminal Records Check and Vulnerable Sector Search as required.
• Availability for flexible hours and adherence to safety protocols including PPE use.

Attention to detail, physical mobility, and safe body mechanics are essential.
