JOB POSTING – RAI COORDINATOR

The RAI Coordinator reports to the Director of Care and manages the information technology and compliance related to Resident Care through coordinating the interRAI LTCF process and monitoring of Quality outcomes and CIHI reports. Responsible for the quality of resident care and nursing restorative philosophy and furthermore ensures compliance through coaching and mentoring.

Also, this position is responsible to ensure staff education, orientation and mentoring to support the transfer of interRAI LTCF knowledge within the Home.

Wage Rate:

The starting wage rate is $27.25/hour.

Vacancy:

This is an existing vacancy.

Responsibilities:
  • Responsible for education and software training to all appropriate team members on interRAI LTCF, CAPS, Care Planning, subscale outcome measures, quality indicators, and reports.
  • Distributes and reviews CIHI reports with team members
  • Coordinates the Relias AIS certification of all newly hired registered staff and annual thereafter
  • Schedules and monitors all informatics on residents assessments
  • Coordination and completion of the Resident Assessments (interRAI LTCF, tracking forms, CAPS, and care plans.
  • Collaborates with the interdisciplinary team to ensure care plans are maintained current and reflective of the individualized care requirements and are in compliance with the Fixing Long-Term Care Act, 2021 and Ontario Regulation 246/22
  • Completes assessments and interviews with Residents and the interdisciplinary team members for data collection and completes the interRAI LTCF assessment, CAPs and Care plans.
  • Facilitates Interdisciplinary Plan of Care Rounds on a weekly basis for all residents due for care plan review.
  • Working in consultation with management and interdisciplinary team members to develop processes that ensure data integrity, efficient data collection process, assessment completion, and data utilization
  • Carries out data quality processes and activities to support interRAI LTCF coding accuracy and its timely completion
  • Generates and evaluates interRAI LTCF and CIHI reports.
  • Discusses findings with department heads, committees, and other meetings as required, and uses them in clinical and operational decision-making.
  • Participates in the development and evaluation of the annual Quality Improvement Plan.
  • Participates in quality initiatives resulting from interRAI LTCF CIHI indicators
  • Required to understand the nature and meaning of quality indicators used by the Home and to understand which indicators are publicly reported.
  • Required to participate in the improvement of the indicators and achieve satisfactory results.
  • Coordinates the data submission process to the Canadian Institute for Health Information (CIHI)
  • Follow up with rejections and makes corrections accordingly.
  • Keeps CIHI Data Submission files and reports in an organized manner
  • Responsible to keep up-to-date with ongoing education by participation in any outside interRAI LTCF, CIHI, and HQO training sessions
  • Ongoing staff development and mentoring to in-house team members on all shifts.
  • Completes all assigned training required as per the Fixing Long-Term Care Act, 2021 and Ontario Regulation 246/22, AIS
  • If directed to by the Director of Care or Manager of Clinical Informatics, takes the lead of the Nursing Restorative program in the Home
  • Mentors and coaches the Nursing Restorative Therapists
  • Communicates any concerns of nursing restorative to the supervisor (Director of Care, or Manager of Clinical Informatics).
  • Orientates new team members to the philosophy, policy, and criteria of Nursing Restorative Care
  • Participates in the interdisciplinary plan of care rounds and discuss who would benefit from nursing restorative
  • Participates in and implements learning which develops one’s own emotional self-awareness and a person-centered approach towards others.
  • Participates in Joint Manager of Clinical Informatics meetings
  • Other duties as assigned and or indicated in Job Task Inventory
Qualifications:
  • Registered health professional licensed under the Health Disciplines Act registered with a regulatory authority in a Canadian jurisdiction, other than Ontario, and holds in that jurisdiction the equivalent of a certificate of registration authorizing independent practice in Ontario as of right exemption Bill 60
  • Well-developed written and oral communication skills
  • Knowledge and experience with adult education strategies/facilitation
  • Ability to identify the training needs of the team and individuals
  • Proficient computer skills and strong problem-solving abilities
  • Ability to work with and relate to interdisciplinary team members and other internal stakeholders
  • Knowledge of interRAI LTCF – AIS certified as RAI Educator; if not must be completed successfully as soon as possible, i.e. within 2 weeks
  • Skills and knowledge in trending and analyzing data
  • Knowledge of Quality Improvement
  • Minimum of two (2) years of experience in a long-term care setting.
  • Minimum of two (2) years supervisory experience desired
  • To show evidence of being focused on people’s feelings when interacting and knowing how to turn a task into a positive social interaction.
  • To undertake and meet all legal requirements within a context of promoting individuals’ rights and measuring/reducing elements of risk.
  • Must provide a complete and current (within six months) Vulnerable Sector Check (including a Criminal Background Check) or be willing to obtain one
  • As a condition of employment, candidates are required to submit documented results of TB testing (within the last six months or within 14 days upon hire) or a chest X-ray (within the last year), as per Public Health requirements
  • Two supervisory references required
  • Provide proof of all required vaccines. The Employer strongly recommend to “stay up to date” with the recommended vaccines. Vaccination requirements are subject to changes as determined by the Employer.
