A community health organization in Toronto is seeking a qualified Social Worker (MSW) to provide trauma-informed care and counselling to clients facing various health challenges. The ideal candidate will possess a master's degree in social work and experience in community services, along with fluency in Cantonese and/or Mandarin. Responsibilities include client intake, crisis support, and developing service plans. This position emphasizes a collaborative approach within a multi-disciplinary team to enhance community health.
South Riverdale Community Health Centre is a non‑profit, multi‑service organization that provides primary healthcare, social and community outreach services with an emphasis on health promotion and disease prevention primarily to people of East Toronto. Our mission is to improve the lives of people that face barriers to physical, mental, spiritual and social well‑being. We do so by meaningfully engaging our clients and communities, ensuring equitable access to primary health care and delivering quality care through a range of evidence informed programs, services and approaches. As a leader in community health, our locally and internationally recognized community services include cutting‑edge primary health services, health promotion, harm reduction, environmental health, community food centre and population‑based community programs for marginalized peoples. As a member of a multi‑disciplinary agency, the Social Worker - MSW will provide therapy/counselling, crisis support, case management, group facilitation, as well as participate on community development initiatives. The focus of this role is to offer trauma‑informed care to clients with multiple chronic conditions impacted by the social determinants of health. Language capacity and ability to support the Cantonese and/or Mandarin speaking, as well as English speaking communities, is a requirement. This position is expected to work onsite.
